The Cursed to Live head is a mission-tied cosmetic that adds a grim, undead look to your Vault Hunter. It’s not a random drop: you earn it by completing a specific side mission in Levaine’s Electi questline after a short chain of prerequisites. Start The Mole’s Gambit in Carcadia

Once the prerequisites are done, Levaine will offer The Mole’s Gambit. The mission culminates in a confrontation with Willem; defeat him, then return to Levaine to turn in the quest. Upon completion, you receive the Cursed to Live head cosmetic in your character’s inventory.

Tip: If The Mole’s Gambit isn’t appearing, confirm you’ve finished “A Traitor Within” in the same playthrough and check back at Levaine in Carcadia.

Equip the head cosmetic

  • Open the character screen and go to customization.
  • Select the Head slot and choose “Cursed to Live.”
  • Apply and back out to save your selection.
