The Clinic and the Monitoring Stands are base structures in Palworld that keep your working Pals healthy by cutting SAN (sanity) loss and blocking new illnesses. The Clinic runs on the Medicine Production work level of the Pal you assign, while Monitoring Stands add a flat passive reduction plus adjustable work intensity. Knowing the exact thresholds tells you which structure to build and which Pal to staff it with.

Quick answer: Build a Clinic (Level 24) and assign a Pal with Medicine Production Level 8 or higher to prevent every base illness. To reach the full 50% building-sanity and 15% overall-sanity reduction at work level 10, you need the Ancient Clinic (Level 73); the standard Clinic caps at 33% and 10% around work level 6.

What the Clinic does and how to build it

The Clinic is a basic medical facility that manages the health of Pals working at your base. Assigning a Pal with Medicine Production suitability reduces SAN depletion across the base and prevents new illness and injury from appearing. It does not cure conditions that a Pal already has, so treat it as prevention rather than a hospital.

You unlock the Clinic at Level 24 for 2 Technology Points. Building it requires the materials below.

Once placed, assign any Pal with Medicine Production work suitability and the Clinic begins working automatically. Its sanitation meter shows medical plus-icons that fill based on the assigned Pal’s Medicine Production level. The standard Clinic displays three green plus icons; the Ancient Clinic displays five.

Illness prevention by Medicine Production level

The Medicine Production level of the assigned Pal decides which illnesses can no longer develop at your base. Each level blocks one additional condition, and level 8 upward prevents all of them. This applies even when Pals are starving or stressed, but again, it only stops new illnesses from starting.

Note: A Pal that already carries a status such as Ulcer keeps it until treated separately. A Level 10 medicine Pal in the Clinic stops the next case from appearing, but the sick Pal in your Palbox stays sick.

How the Clinic reduces sanity drain

The Clinic applies two separate sanity effects that both scale with the assigned Pal’s Medicine Production level. The first reduces the sanity drain coming from work structures. The second is a passive base-wide reduction that applies to every Pal.

The standard Clinic cannot use the top rows of that table. Its building sanity reduction caps at 33% (roughly work level 6) and its overall sanity reduction caps at 10% (also around work level 6). To push those numbers to the full 50% and 15% at work level 10, you must use the Ancient Clinic.

Clinic vs Ancient Clinic

The Ancient Clinic is the top-tier version and offers significantly greater protection than the standard Clinic. It unlocks much later and costs advanced materials, so the standard Clinic remains the practical choice through most of the game.

Monitoring Stands: passive reduction and work modes

Monitoring Stands add a flat, passive sanity drain reduction on top of the Clinic. The High Quality Monitoring Stand gives 5%, and the Ancient Monitoring Stand gives 15%. These stack with the Clinic’s effects, so a high-tier setup combines both.

Interacting with a Monitoring Stand also lets you set how hard your Pals work. Harder settings raise work and move speed but drain sanity faster and increase hunger. The Relaxed setting is the only one that restores sanity over time.

Sanity drain from work stations

Different work structures pull sanity from your Pals at different rates, which is why the Clinic’s building-sanity reduction matters most on heavy production bases. The Gold Coin Assembly is by far the harshest at -0.5, while the Ranch drains none.

The Clinic and Monitoring Stands are not the only ways to hold sanity steady. A few Pals, structures, and foods add their own reductions that stack into your base setup.

The dawn illness RNG check

Even with a standard Clinic staffed, Pals rarely get sick during normal working hours. The one gap is a small random chance for sleeping Pals to catch a low-tier illness, such as a Sprain, during their waking-up animation at dawn.

You can bypass this entirely by using a Pal that never sleeps. A Pal with the Insomnia trait or a Dark-type Pal skips the wake-up animation, which removes the check. That makes the Clinic’s remaining job almost purely about sanity management rather than illness.

For most bases, a standard Clinic with a Level 8+ Medicine Production Pal handles illness prevention completely, and a High Quality Monitoring Stand on Relaxed or Normal keeps sanity stable. Save the Ancient Clinic and Ancient Monitoring Stand for late-game production hubs running structures like the Gold Coin Assembly, where the extra reduction to 50% building sanity earns its steep material cost.
