Chapter 3 of The Sinking City 2 drops you into Akeley Memorial Hospital, and one of the more rewarding side detours there is a metal gate in the Storage room on the -1F level. It is held shut by a four-digit padlock, and the digits are split across four separate notes scattered around the hospital’s second floor. You can work them out properly through the Mind Palace, or you can skip the legwork and dial the number in directly.
Quick answer: The combination is 1308. Rotate the dials on the padlock to 1-3-0-8 and confirm to open the gate.
What you need before you can reach the padlock
The lock is not the first obstacle here. The Storage room door itself is rusted shut, and opening it requires the Anti-Rust Acid found in the Prosthetics Workshop on 2F. Use the acid on the door, step inside, and the combination lock is on the gate beyond it.
Timing matters a little. The four notes and the lock all stay reachable right up until you use the charged prism to clear the barrier in the hospital. Wait longer than that and some of the note locations become more awkward to get back to, so it is worth sweeping 2F first.
Where the four digit notes are on 2F
All four clues sit on the second floor of the hospital. Picking them up is optional if you already know the code, but collecting them completes a deduction group, which is worth doing while you are in the area.

Note: you do not need the deduction group finished for the code to register. Entering 1308 works the moment you can reach the lock, even if the notes are still sitting on 2F.
What is behind the Storage gate
The room stocks ammo and crafting materials, plus Doyle’s Cold Blood, an attachment for the submachine gun. Opening the gate also pops the “The Vault” achievement and trophy, which is the clearest confirmation the puzzle registered as solved.
If the lock will not open
There are only two realistic reasons the gate stays shut. Either you have not used the Anti-Rust Acid on the Storage room door yet, so you are standing at the wrong barrier entirely, or the dials are not all reading the intended digits. Check each dial individually before confirming, since it is easy to leave one a position off after spinning through the numbers.
Clearing this early saves a backtrack later. The intended flow has you finding the notes across 2F before you ever see the padlock, but there is nothing stopping you from grabbing the acid, walking down to -1F, and taking the submachine gun attachment on your first pass through the lower floor.
