Unholy Vitality is one of the few Witchcraft abilities in The Blood of Dawnwalker that quietly keeps Coen alive without asking for a button press. It regenerates health after you use an Active Ability, and at higher ranks it turns critical hits into healing. Getting it to 4/4 is a supply problem more than a combat one, because every rank consumes another copy of the Unholy Vitality Manual.
Quick answer: Buy the Unholy Vitality Manual from the Matriarch Witchcraft Vendor in Uriash Village after finishing Song of the Mountain, then keep buying or farming copies from the Peddler of Wonders, Mevasser, and the Sanzhana Merchant until you have four — one per rank — and spend them at any Shrine.
What Unholy Vitality does at each rank
The ability is day-only, like the rest of the Witchcraft tree, and it works passively once it sits in the Active Abilities panel. Rank 4 is the payoff, since it removes the health cost of casting entirely while you are below 30 percent health.

Buying is the only repeatable method, which makes vendors the backbone of a 4/4 push. Four merchants carry the book, and because there is no hard cap on how many manuals exist in a playthrough, you can return to the same shop after stock refreshes.

Merchant inventories are partly randomized. The Peddler of Wonders in particular may need several visits before the Witchcraft manuals appear, so treat an empty stock page as temporary rather than a dead end. Prices swing too — the Matriarch has been seen asking 1,182 gold for a single copy, and the Bewitching Influence perk lowers what merchants charge.

Where the manual drops in the open world
Two world sources are fixed rather than random. Leonica, She Who Speaks with the Devil drops the manual when defeated, and the Haunted Site activity southwest of Eerie Knoll Shrine rewards a copy on completion. Clearing both gives you two ranks without spending gold.
Beyond those, Witchcraft manuals of all kinds show up in Scouting Tower chests, in supernatural creature caves, nests, and lairs, and as rewards from Ancient Circles, Haunted Sites, and Will-o-the-Wisps. Defeated enemies can also drop them. Which manual you get is not fixed, so these are a supplement to vendor shopping, not a replacement for it.
There is also a quest route. Running the Echoes of the Silent Bells questline out to St. Tyna’s Convent and beating the boss at the end can hand over a Witchcraft manual, and clearing it turns Anca at the Crossroad Inn into another Witchcraft merchant worth checking.
Unlocking and ranking up Unholy Vitality
Owning the manual is only half of it. Head to any Shrine and interact with it to open the development tree, then find Unholy Vitality in the Witchcraft branch. Each rank costs skill points, a time slot from the in-game clock, and one manual.
Skill points come from levelling up and from quest rewards, so a manual surplus does not help if you are short on points. Plan on four separate Shrine visits or one visit with four manuals and enough points banked.
You will know it worked when the node in the Witchcraft tree fills to 4/4 and the tooltip shows the rank 4 text, including the line about no health cost below 30 percent health.
Why the healing is not triggering
Two things stop Unholy Vitality from working even when it is fully ranked. It only functions while it is equipped in the Active Abilities panel — sitting unlocked in the tree does nothing. And it is a Witchcraft ability, so it is unavailable at night when Coen shifts to his vampiric kit.
Note: the rank 3 and rank 4 lifesteal only applies to critical hits on enemies currently affected by a Witchcraft ability. If nothing is hexed, you will still get the flat health regeneration but none of the crit healing.
If gold is tight early on, take the Haunted Site and Leonica copies first and save the vendor purchases for the last two ranks, when your purse and your merchant discounts have both caught up.
