The Medical Centre – Different section of Silent Hill: Townfall locks Simon behind a blood transfusion system that only releases once real blood has been pushed through it. Getting out means pulling five blood bags out of the wards, hanging three of them in the right wall slots, and then routing three compatible donors into the container at the middle of the Transfusion Door.
Quick answer: Hang Sarah Watson in the 2nd left slot, Matthew Clark in the 3rd left slot, George McEwan in the 4th right slot, then connect H.M., T.F., and S.W. to the Ordell container.
All blood bag locations in Medical Centre – Different
Five bags exist in this area, and they are not all used the same way. Alan Duncan and Lesley Greer are keys in everything but name, feeding the transfusion machines that raise the shutters into Ward S.W. and Ward D.P. The other three are the ones you keep for the wall.
At the outset, take the Medical Centre – Different Map lying centrally in your starting room. The wards, basements and vent routes are difficult to memorize, while the map identifies the Blood Transfusion Machine you will revisit.

Use the grate to enter the southeasternmost room, then crawl along the vent. Alan Duncan’s Blood Bag is suspended from an IV pole there, and the tape recorder nearby gives a clue about its destination.
Return to the door marked S.W. and slot Alan Duncan’s bag into the transfusion unit beside it. Hit the transfusion button and the shutter gate rolls up, opening Ward S.W.
Inside the ward, turn left into Room 05 and take Sarah Watson’s Blood Bag from above the covered bed. The recording here mentions head trauma and a pulse of 90 — that pairs her with a specific slot later, so it is worth remembering.
Take the staircase north of Room 05 down into the basement. A large enemy patrols from this point on and kills Simon instantly on contact, so stay in stealth rather than fighting, and keep IV Bags on hand to recover if it catches you.
Collect the Ward W Master Key from Room 04 in the basement, then use it to open Room 18. A ladder inside climbs into Room 12 of Ward S, where Lesley Greer’s Blood Bag sits next to an IV stand.

Because the large monster is now patrolling the floor above, exit Greer’s room via the vent on the left instead of using the door. Then reach the right-side ladder beside Room 13 and descend into the safer starting area.
Go into Room 02 and check the elevator shaft, where a body remains wedged around a blood bag. Activate the elevator control to lower the body to the basement, making the bag accessible later.
Outside Room 06, use the vent at the eastern corner to reach Room 07, then descend into the basement. George McEwan’s Blood Bag lies beside his body in Room 10; taking it also gives you the Ward P Master Key.
Crawl through the vent in Room 02, take the stairs near Room 05 back upstairs, and activate the valves in Room 05 and Room 10 while avoiding the patrolling enemy. With both valves running, climb the ladder between Rooms 01 and 09 to get out.
That ladder breaks the moment you use it and seals Ward D.P. permanently, so sweep the ward for items and collectibles before you climb.

Back at the transfusion wall, the three remaining bags each match a patient assessment clipboard mounted near the stations. Read the injuries, then count the slots along the wall the clipboard belongs to.

With all three hanging correctly, pull the transfusion switch next to the locked metal door. Blood runs through the frame and the heavy double doors grind apart, exposing the circular dial lock on the wall beyond.
Look for the switch on the wall to the left of the doors, rather than on the doors, when no prompt appears.
Transfusion Door puzzle solution (H.M., T.F., S.W.)
The Transfusion Door is a plumbing problem wearing a medical chart. Six outer nodes carry patient initials — D.H., S.W., R.D., M.C., T.F. and S.H. among them — and the concentric rings between them hold short lengths of red pipe. Turning a ring shifts its segments, and a line only counts once it runs unbroken from an outer node into the Ordell container at the center.
You are not meant to connect everything. The machine needs three donors whose blood can pool together without clashing, which fixes the answer rather than leaving it open.
Walk up to the dial and open the puzzle view. The container sits at the middle, ringed by the patient initials on the outer edge.

Ignore every node except H.M., T.F. and S.W. Any other combination clashes, and pushing paths to the extra initials only makes the rings harder to align.
Select one ring at a time and rotate it until the pipe segments meet end to end. Work one donor from the outside inward, then recheck the earlier lines, because the ring you just turned may have cut a path you already finished.
After all three feeds connect to the center, interact with the Ordell container by selecting it. Blood moves inward, the mechanism locks itself, and the door opens automatically.
Each bag around the room shows a patient’s initials together with that person’s blood type. Read those labels to identify the correct trio instead of trying to guess from the dial.
How to confirm the Transfusion Door is solved
You will know the alignment is right when uninterrupted red lines run from all three outer nodes into the center. The container fills, the heavy door opens automatically into a dark barred corridor, and passing through it creates a save.
No response to interact means one or more of the three feeds has not reached the container. Cancel the highlight, then follow every route inward from its node to locate the ring where a pipe segment fails to connect.
The other dial lock: Haldane Hotel bathroom door (18, January, 1996)
Townfall uses a second rotating-wheel lock much earlier, on the bathroom door in the room you reach after calling 100 from the Town Square phone booth in St. Amelia’s Shorefront. It looks like the Transfusion Door but takes a date instead of a flow path, entered from top to bottom. On Normal difficulty the answer is 18, January, 1996.
Release the lock, then rotate the topmost outer wheel until it displays 18. Tune in the second yellow signal and save it on the CRTV; its opening frames show this number.
Set the innermost wheel to 1996, the year printed on the room service card on the desk. The lock disengages the moment the third value lands and the Open prompt appears on the door.
The two locks follow an identical rule: look for the solution in the room where you are standing, such as the year on a room service card or the blood type printed on a bag hanging from the wall.
