The Plague Doctor sits behind a barred cell under your house, and he is the gatekeeper for the Anatomy and alchemy branch of the tech tree in Graveyard Keeper 2. Until you reach him, Dream Dust, the Alchemy Lab, and your first zombies all stay locked off. Getting to him is not a puzzle — it is a specific chain of conditions, and one of them is a friendship number most players miss.

Quick answer: Reach at least 10 friendship with Bally after meeting her through Januarius in the Village Tavern, then speak to Gerry the skull at home to open the eastern basement hallway.


What the Plague Doctor unlocks

He is the entry point to the Anatomy and alchemy section of the tech tree. Everything downstream of that branch — Dream Dust production, the Alchemy Lab in the Abandoned Tower, and later the Reanimation technology that lets you make more zombies — runs through him. If your tech tree is stalled on that side, this is why.

The Good Fellow entry spells the goal out directly: to summon a fairy you need Dream Dust, and to get Dream Dust you have to go into the dungeon and find the Plague Doctor. Dust in the Kitchen runs alongside it with the objective to figure out how to summon the Fairy. If those entries are not in your log yet, keep pushing the main story — the cell will not open early.

How to reach the Plague Doctor

Find the trapdoor first so you know the route. It sits in the back room of your house, and it drops you into the Writing Basement. From inside the basement, walking west and up the stairs brings you back into the house, which is the same path in reverse.

If the hatch is still sealed, craft a toolbox and use it there. Toolboxes need Bronze Details and Planks, so unlock basic woodworking and bronze work before you make the trip.
Leave the church yard and head east to the Village. Open the map if you are unsure of the route — the path runs straight east from your base.
Enter the Village Tavern and talk to Januarius at the counter. He introduces you to Bally. This introduction is mandatory, not optional flavor.
Build Bally’s friendship to at least 10. The conversation that moves the chain forward will not fire below that threshold, which is the single most common reason players get stuck at this point.
Run back west through town to your home base and speak to Gerry the skull. He is the one who opens up the basement area beyond the writing room.
Take the trapdoor down into the Writing Basement again and walk east through the hallway that is now unlocked. At the end of it is the barred cell with the Plague Doctor inside, and a cutscene plays when you arrive.

Clearing the rubble to free him

Seeing the Plague Doctor is not the same as unlocking him. He is pinned behind rubble, and you cannot talk to him properly until it is gone. Clearing it costs a second toolbox, so bring the Bronze Details and Planks for two of them on your first trip down rather than climbing back out for materials.

Craft both toolboxes before you go anywhere near the basement. One opens the hatch, the other removes the rubble, and the walk back to your workbenches wastes a chunk of the in-game day.


How to confirm the Plague Doctor is unlocked

You have it right when the conversation with the Plague Doctor triggers after the rubble is cleared and the Abandoned Tower becomes available at your base. Leave the house and head north to find it, then open it with your medallion using the items already in your inventory.

  • The rubble in the basement cell is gone and the Plague Doctor is talkable.
  • The Abandoned Tower north of the house can be entered with your medallion.
  • The tower’s Alchemy Lab, including the Study Table and Laboratory table, is now usable.
  • Your quest log advances toward producing Dream Dust for him.

From there, the immediate follow-up is Dream Dust. Research one green item and one blue item at the Study Table — blood and water are the easiest pair — then combine the results at the Laboratory table and hand the Dream Dust to the Plague Doctor. That cures his cough, opens the cellar below the lab, and puts your first two zombies in your hands.
