Deep inside the cult’s compound in Clive Barker’s Hellraiser: Revival, Aidan runs into a locked door covered in a fleshy mouth. There is no handle and no obvious switch nearby, only a human skull sitting on a desk with pins and barbed wire jammed into it. That skull is the lock. Press the pins in the right order and it opens to reveal the Tongue Key you need for the door.

Quick answer: Press the pins in this order — right side, top-back, left side, then top-center. The skull opens and gives you the Tongue Key, which unlocks the mouth door.

Where the skull puzzle and mouth door are

You reach this room shortly after the first save point, once Aidan has changed into a fresh set of clothes. From there you sneak down a hallway past two patrolling cultists. Crouching in the dark lowers your chance of being spotted, so stay low until you slip through into the next room.

Inside, the door with the fleshy mouth sits on one wall, and the skull wrapped in barbed wire and pins rests on the desk. The door stays sealed until the skull hands over its key, so start with the desk. Interact with the skull to pick it up and enter the pin mini-puzzle.

Look around the hallway and side rooms first. There is loot to grab, including coins from a cabinet and a collectible note on the desk, before you commit to the puzzle.

Correct pin order for the skull puzzle

The positions below assume the skull is facing you, which is how it sits when you first pick it up. You can rotate it to inspect the crown and the back, but the solution only needs four presses in sequence.

Once the fourth pin goes in, the mechanism disengages and the skull’s jaw drops open. The Tongue Key is sitting inside the mouth, resting on a preserved tongue. Press the interact button to add it to your inventory.

Opening the mouth door with the Tongue Key

With the Tongue Key in hand, walk up to the fleshy mouth door and interact with it. Choose the Tongue Key to feed it into the orifice, and the door unseals. You will know it worked when the mouth opens and the path forward becomes available, letting Aidan move on to the next part of the cult compound.
