Phantom Blade Zero ends its prologue by putting two hearts in front of Soul. Each one is a way to keep him alive, and each comes at a cost. If you were waiting for a choice screen, it never appears.

Quick answer: No. At the end of the prologue, the Flesh Sculptor describes both hearts, but no menu or prompt appears. The scene picks the Tainted Heart for you, which is why Soul has 66 days to live.

What the Flesh Sculptor offers

The scene plays right after you beat Zuo Shang, the Order’s Shadow Sovereign, at the close of the prologue chapter, “DIVERGING PATHS”. You don’t need anything special to reach it. Finishing that duel is enough. A hooded figure with red eyes, the Flesh Sculptor, holds out two hearts. One is metal and looks mechanical. The other is dark and veined.

The Flesh Sculptor explains the trade-off in his own words:

“The first is the Hollow Heart. It will restore your heart… but strip away all emotion and desire. You would live by reason alone.”

“The second is the Tainted Heart. Venom will keep your heart beating… but in sixty-six days, it will claim your life.”

Why you end up with the Tainted Heart

The Flesh Sculptor’s next line suggests the outcome was decided before Soul ever spoke: “Just as I predicted… hahahahaha!” The prologue then closes on a close-up of Soul’s eye and the card “DIVERGING PATHS / END OF CHAPTER”.

What the Tainted Heart means: Days Remaining

The story jumps to “Present Day”. Soul rides a cart with Old Man Gen, then fights The Hunt, Hunter of the Order. A cutscene labeled “One Day Before” follows and explains what Soul is chasing. The Flesh Sculptor has heard of “a Heart in Pangzhen… a heart that can remake flesh… raise the dead.” His offer is direct: “Bring me that Heart, and I will make it yours. Then you’ll live beyond sixty-six days!”

You can check the result on the Chapter 1 card, “AGAINST THE FLOW / Follow the path upstream”. A “Days Remaining: 66” counter sits in the bottom right corner. That number is the Tainted Heart’s deadline.

Soul in a wide straw hat faces a mossy forest stream with a body hanging from a tree ahead.
Soul stands on a forest streambed as the quest “Against the Flow” begins with the objective “Follow the path upstream” and 66 days remaining.

In 66 DAYS mode, each death costs one day (more in what happens when you die). If the counter runs out, the run ends with “Death’s End: Lost to the Wulin”.

In the prologue, the Hollow Heart is not something you can take. What the scene leaves you with is the Tainted Heart, the 66-day clock it starts, and a reason to head for Pangzhen.
