Your class in Animal Hospital decides how each shift feels before you ever check in a patient. Every role hands you a passive perk that leans into one of three things, more Sanity, more inventory or utility, or a weapon to deal with anomalies. There are 10 classes in total, and every player starts locked into the free Intern until they earn enough currency to buy something else.
Quick answer: Buy classes from the Class Shop inside the yellow circle in the lobby, right in front of the ambulance. Most classes cost Animal Coins earned from completing shifts, while Head Nurse and Secret Agent require Robux. Each class has three upgrade levels that improve its perk as you complete objectives.

Every class also has three levels. You raise a level by completing specific objectives during play, and each level makes the perk noticeably stronger, usually with extra Sanity or added uses of a starter item.
Every class explained
Intern

The free default. Intern starts each shift with bonus Sanity and nothing else to track, which is exactly what a new player needs while learning the anomaly and treatment loop. Once you have Animal Coins to spend, you will want a class with real utility.
Nurse

A cheap support pick that adds inventory slots so you can carry more treatment items in one trip. Extra space stays useful in every shift, but the perk is small on its own and works best when you are ferrying supplies for a team.
Secretary

Secretary rewards working the reception desk, recovering Sanity each time you check in a patient. Higher levels stack bonus Sanity on top. It suits players who like to man the front and process arrivals rather than run laps to the treatment rooms.
Paramedic

Paramedic spawns you with a Large Speed Cola, a movement buff you can drink several times. Faster travel makes it easier to juggle several patients at once, and at Level 3 the cola regenerates a use each round so you rarely run dry.
Psychologist

A high-risk pick. Every Sanity change is doubled, so coffee and treatments restore twice as much, but anomaly hits drain twice as fast too. Later levels add a chance to ignore Sanity loss entirely. It rewards players who track their meter closely.
Doctor

Doctor gives back Sanity every time you heal a patient, which pairs perfectly with a role that lives inside the treatment rooms. It is a strong pick for anyone who lets teammates handle check-ins while they cure patient after patient.
Security

Security starts with an X-Taser to shock anomalies and crazed patients before they spiral into a bigger threat. It shines in teams where one person handles danger while others treat and check in. Level 3 refills a taser use each round.
Head Nurse

A Robux class built around inventory. Head Nurse grants +3 slots from the start, adds bonus Sanity at Level 2, and at Level 3 lets you begin each round holding all items. That makes it a strong all-round support role for group play.
Surgeon

Surgeon is essentially an upgraded Doctor. Each heal restores Sanity and grants a temporary speed boost, which stretches to eight seconds at higher levels. The mix of Sanity and movement makes it one of the best coin-only classes for treating patients.
Secret Agent

The most expensive class and the only one that arms you with a gun from the very start. Secret Agent lets you shoot down anomalies without hunting for a weapon mid-shift, and higher levels add uses, per-round regeneration, and bonus Sanity.
Best classes to pick
If you are earning your way up with Animal Coins, Surgeon is the strongest choice for most players. Recovering Sanity and gaining a speed boost every heal keeps you rewarded for the core objective, making it far more sustainable than Doctor, which it effectively replaces. Doctor is still a fine cheaper stepping stone, and Paramedic works well if you only care about raw movement speed.
If you are spending Robux, Secret Agent is the best overall. Starting every shift with a gun gives you a reliable answer to anomalies without scrambling for items, which takes a lot of pressure off while you manage patients at the same time. Head Nurse is the pick if you would rather support a team with a full inventory.
Skip sinking early coins into Nurse or Secretary. Their perks are too minor to change how a shift plays out compared with what Doctor and Surgeon offer later. For brand-new players, the free Intern and its Sanity cushion is the safest place to start while you learn the loop and bank coins for something stronger.
