Two cat spirits are hidden inside the Akatori Monastery, and finding both is the only way to unlock the Akatori Monastery Spirits achievement. One sits in the early caves, the other waits inside the Burning Akatori Monastery. The catch is timing. The first cat has to be picked up before the area seals shut for good.
Quick answer: Grab Cat 1/2 in the lower monastery caves at the start of the game by hitting a lever to open a gate, then collect Cat 2/2 in the Burning Akatori Monastery by dropping down a vertical shaft into water and swimming right into the cave.
Cat 1/2: Lower monastery caves (collect early)
The first cat spirit is tied to the opening stretch of the game. Once you progress past this point, the area locks permanently and you can no longer return for it. If you skip it here, the achievement becomes unobtainable on that save.
Note: Because this cat cannot be reclaimed later, treat it as a priority the moment you reach the caves rather than something to backtrack for.
Cat 2/2: Burning Akatori Monastery
The second cat spirit appears in the Burning Akatori Monastery, reached later in the run. Your counter will read “Cats Found 1/2” once you arrive with the first cat already in hand, so you know only this one remains.

Confirming the Akatori Monastery Spirits achievement
Collecting both cat spirits unlocks the Akatori Monastery Spirits achievement, described as collecting all Akatori spirits in the Akatori Monastery. When the second cat registers, the achievement pops on screen. It is a rare unlock, sitting at around a 0.1% completion rate.
If the achievement does not trigger, the most common reason is a missed first cat. Since the lower caves seal off once you move past the opening area, a save that advanced without it will show only 1/2 with no way to raise the count. Starting a fresh run and grabbing Cat 1/2 immediately is the only fix in that case.
