The AFK farm in Animal Hospital works because the shift score rewards prevented anomalies, and rejecting patients at the front desk counts toward that number. By closing the reception shutters after most patients are checked in, every visitor that walks up and leaves is logged as a rejection, so gold cat coins keep stacking while you stand still.

Quick answer: Buy the Extra check-in window upgrade, check in patients until only 1 remains, press the red SHUTTERS button to close the reception, then go idle. Rejected patients count as prevented anomalies. Return within about 20 minutes, treat the final patient, and collect the reward.

Why closing the shutters farms coins

Each shift grades you on how many anomalies you prevent. The SHUTTERS button on the left of the desk is normally used to reject anomaly patients, which lowers the shutter and sends that “patient” away. When the shutters stay closed, every visitor that approaches is turned away the same way, and the Anomalies Prevented counter climbs without any input from you.

More prevented anomalies means a higher Hospital Performance rating and a larger coin payout at the end of the shift. That is the entire basis of the farm. You leave one patient unchecked so the shift does not auto-complete, then let the closed reception rack up rejections.

Setup before you go idle

Two things maximize the farm. The Extra check-in window upgrade doubles how fast rejections register, and your Sanity meter sets a hard limit on how long you can stay away.

How to run the AFK farm

Go to the nurse counter and press E to buy the Extra check-in window for $190. A “bought second check-in” notice confirms it, and your money drops accordingly.

Walk to the main lobby check-in desk and start checking in patients. The CHECK IN PATIENTS counter ticks down each time. Keep going until it reads CHECK IN PATIENTS 1 so your reward stays in the Good or Excellent range.
Move to the wall next to the window and press the red SHUTTERS button. The metal shutters close and display “OUT OF SERVICE.” This is where the farm runs.

Stand idle. Animal patients walk up to the closed shutters, show a dialogue bubble, and leave. Each one is logged as a prevented anomaly. Keep the session to around 20 minutes, so Sanity does not run out.
Return and press the red button again to open the shutters. Check in the final patient so the counter reads CHECK IN PATIENTS 0. Then treat every checked-in patient, for example processing DNA results at the room terminals.

Handle any emergency that appears. Shift 4 always triggers one, so keep a fire extinguisher ready to put out patients on fire and clear the critical-patient timer. Make a coffee in the breakroom to top up Sanity, then speak to Dr. Harlow to receive your bonus.

Tip: To stay away longer than 20 minutes, run an auto clicker so Sanity is managed and you are not booted for inactivity.


How to confirm the farm worked

The SHIFT REPORT clipboard at the end of the shift is your verification. A successful farm shows a large Anomalies Prevented total and a coin reward to match. One clean run produced these numbers.

The higher your Anomalies Prevented count, the more cat coins you earn, which is what makes idle rejection worthwhile. If the report shows a low prevented total, either the shutters were not closed long enough, or the shift ended early. Leaving exactly one patient unchecked before closing the shutters keeps the shift open so the counter can keep rising, and treating your checked-in patients correctly avoids Animal Deaths that would drag down the performance rating.
