Coins in Animal Hospital pile up fastest when you turn the game’s rejection mechanic against itself. Instead of treating patients one by one, you can close the reception shutters and let a stream of visitors arrive, get turned away, and count as prevented anomalies. Each rejection pays out, and because the loop runs on its own, you can step away and let the counter climb.

Quick answer: Check in patients until the CHECK IN PATIENTS counter drops to 1, then close the reception shutters with the red SHUTTERS button. New patients keep spawning, hit the closed window, and leave as prevented anomalies that award coins. Run an auto-clicker every 20–30 seconds near the window so Roblox’s idle timer never expires.

Why rejecting patients pays coins

Every patient who walks up to a closed reception window and leaves without service registers as a rejected visitor, or a prevented anomaly. Those events award coins and money, and they keep firing as long as patients keep spawning.

The trick that keeps patients coming is the check-in requirement. The green CHECK IN PATIENTS sign on the reception wall shows how many visitors the shift still expects. As long as that number sits above zero, patients continue to arrive and try to check in. Keep it at 1 and never complete that final check-in, and the game keeps sending people to the window indefinitely.

Because the shutters are down, none of them get served. They ring the bell, complain, and walk off, each one adding to your total. Sanity stays intact, and the shift never fails, since you are not neglecting anyone who actually made it inside.

Set up the shutters farm

Buy the Extra check-in window upgrade at the reception desk for $190. It lets more patients spawn at once, which pushes you toward the per-shift cap faster. This step is optional but speeds the whole run up considerably.
Open the shutters and check patients in one at a time. Sign the forms on the clipboard, register the patient on the computer, and print their badge from the printer, pressing E at each station. Repeat until the CHECK IN PATIENTS counter falls from its starting number down to 1.
Stop with one patient left. Do not check in the final one. Walk into the office room next to the lobby and press the red SHUTTERS button (the one with the sticky note reading “Use shutters to reject people”). The window rolls down and displays CLOSED and OUT OF SERVICE.
Take the fire extinguisher from its wall mount in the hallway. The prompt reads “fire ext: 100% extinguish fires.” Keeping it in hand near the closed window means a patient who arrives on fire gets handled automatically when your clicker fires.
Park your character next to the closed window and start an auto-clicker such as TinyTask set to click every 20 to 30 seconds. Each click registers as input, so Roblox never flags you as idle, and it also triggers the extinguisher on any burning patient.

Note: Return the fire extinguisher to its wall mount now and then to recharge it. It climbs back up slowly, so a quick swap keeps it ready for the next fire patient.

Beat the 20-minute idle kick

Roblox removes any player who registers no input for roughly 20 minutes. This timeout is platform-wide, so it applies inside Animal Hospital the same as anywhere else. A character standing perfectly still gets booted before the farm banks much, which is the single most common reason an AFK attempt earns nothing.

The timer resets on any input. That is exactly why the auto-clicker matters. A click every 20 to 30 seconds keeps the session alive without you touching the keyboard, and it doubles as your fire response. Stay on one server the whole time, because rejoining or hopping servers restarts the shift and wipes your progress for that run.

The 100-anomaly cap and collecting coins

The coin reward stops once you reach 100 anomalies avoided in a single shift. With one reception window, hitting that ceiling takes about 1 hour and 20 minutes. The Extra check in window upgrade spawns more patients, so more of them arrive and get rejected in the same span, which reaches the cap sooner.

Once you hit 100 prevented anomalies, the payout dries up for that shift. End the shift or leave the game to collect the coins you accumulated, then start a fresh shift and repeat the setup if you want to keep farming.

Confirm the farm is working

The setup is working when the Anomalies Prevented count keeps rising, your coin total is higher than when you stepped away, and you are still on the same server you started in. If the number has stalled, something in the loop broke. The most likely causes are below.
