There are 46 trophies on PS5 and a matching 45 achievements on Xbox and Steam (no Platinum on those platforms). None are missable, none are glitched, and nothing is tied to difficulty, so the entire list can be earned on the easiest setting.
Quick answer: Finish the story once, clean up every collectible, then start New Game+ and clear it. Farm the Crater Zone to reach level 100, then use the save-and-reload trick to buy all weapon upgrades and unlock every skill. When all 45 other trophies are done, Duty Fulfilled pops as the Platinum.

Trophy count and completion facts
The PlayStation list breaks down into one Platinum, two Gold, 11 Silver, and 32 Bronze. Xbox and Steam carry the same milestones minus the Platinum. Every trophy description is visible from the moment you boot the game, since none are hidden.

The list cannot be finished in a single run. Reincarnated only pops after you clear New Game+, and a handful of other requirements are gated behind that second campaign too. New Game+ forces a difficulty called Reincarnation+, which sits roughly at the first playthrough’s Normal setting. If you ran the base game on Easy, the jump will feel more noticeable.
Three things specifically require New Game+: some of Emma’s and Koo’s later skills, one final blade that only enters your inventory after the first ending, and speaking to 30 Keeper Golems (you cannot find that many in one campaign). Everything else can be collected on the first run.
Note: After beating the final boss, save the game. That file gains a star, and you can keep exploring every area to mop up before you use it to launch New Game+.
Boss and combat trophies
Most boss trophies unlock automatically as you progress the story. The two grind-based combat trophies (The Sealer and Repose for the Departed) accumulate naturally across two playthroughs, so you rarely need to farm them on purpose.

Story and progression trophies
These pop on their own as you play, or the first time you touch a core system. Life After the End and The End of Reincarnation both trigger at the ending and cannot be missed.

Rapport with Koo and Kagura both cap at level 10, and you climb it fast. Keep talking to them in the Cleanse Walker, clean up Koo in the shower, and let their research projects finish. Research advances through battle experience, which feeds your active project until it completes and rolls over to the next one.

Collectible trophies
Grab paintings, documents, seedlings, and every campsite and shrine as you explore. The first nine areas each hold a single lore painting hidden somewhere in the level. Finders, Keepers is the one collectible that spills into New Game+, since 30 Keeper Golems will not appear in one campaign.

Reaching level 100 with Emma and Koo
After both playthroughs you will usually sit somewhere between level 60 and 70. Level 100 with Emma unlocks The Ultimate Vessel, and level 100 with Koo unlocks The Perfect Pal. There is one reliable farming loop that closes the gap quickly.
Skill mastery: Blademaster, Floral Profusion, and the Giant Tree trophies
Some Blade Arts, Finishers, and skills only unlock during New Game+, so none of the four skill trophies can be earned on the first run. Both Emma’s and Koo’s skill trees are split into two parts, the roots and the tree itself.

At level 100 you will have enough skill points to fully unlock one character’s tree plus most of the other’s. Use the save-and-reload trick to avoid extra grinding. Save the game, spend points to pop Blademaster, reload to get the points back, then spend them again for Floral Profusion. Do the same pairing for Giant Tree Budburst and Giant Tree in Bloom, which only need the root section plus the base level of every tree skill.
Weapon upgrade trophies: Blade Enthusiast and Munitions Master
Both upgrade trophies cost only money, not crafting materials, so the same save trick applies. Blade Enthusiast asks you to take all 21 swords to +3. Every blade but one is available on the first run, and the final sword (Kanzashi, a reward for beating the game) only appears in your inventory during New Game+.
Munitions Master is the expensive one. Each arrow and bolt type upgrades to +14, split across seven element levels and seven quiver levels, and the full set runs roughly 500,000 to 600,000 money. Leave it for late New Game+.
Money trick: Save the game, sell everything you own to raise the cash, buy every upgrade you need, then reload the save to get your money and items back. The upgrades stay unlocked.
Confirming the Platinum
Duty Fulfilled is awarded for collecting every other trophy, with no DLC required. The moment the last of the 45 pops, the Platinum unlocks on the same screen. A clean order that avoids wasted time is one relaxed exploration run to gather collectibles and boss kills, one New Game+ clear for Reincarnated and the gated items, then the Crater farm to level 100 followed by the save-and-reload passes for every skill and upgrade trophy. Do that and the completion list closes out in around 30 hours.
