The appearance window is tied to the server clock, not a respawn timer you can watch tick down. It returns on three fixed days each week, at the same time of day:

The rarest pulls are the Unique-grade pieces. The Artwork decor item sits at the top of this group at a flat 25%. Below it are two Unique armor lines, the Argo set and the Fallen Ancient God set, each weighted by slot. Weapon drops by grade
Weapons drop in four grades, and the rate is the same for each of the nine weapon types: Greatsword, Longsword, Dagger, Bow, Spellbook, Orb, Mace, Staff, and Guard. The Vowed weapons are the Epic tier worth farming for; the Fallen Ancient God weapons are the Unique tier and carry the longest odds on the whole boss.

Which drops are worth chasing
The Vowed weapons are the clearest reason to repeat this fight. They are Epic grade at item level 38, require level 35, and carry three manastone slots plus one theostone slot. They can also be traded until they become soulbound, which gives them value beyond your own character.

Each of those Vowed weapons lands in the upper portion of its weapon list, well clear of the midpoint. They are not top-end gear, though. The strongest greatswords in the game are Ludra’s Blade of Extinction and the Splendent Ebony and Wise Dragon Lord greatswords, which sit far above the Vowed Greatsword’s value.
If you are hunting a trophy rather than a stat line, the Argo and Fallen Ancient God sets are the Unique pieces to target, with the accessories and lighter armor slots appearing most often. The Fallen Ancient God weapons at 2.69% are the single hardest result to force, so plan on many visits across the weekly schedule if one of those is your goal.
