The bell door in Hellraiser Revival sits in an alley near the finale, and it will not open until all five bells are mounted and set to the right symbols. You start with only four. The fifth is hidden behind a short combat route, and claiming it triggers a boss fight before you can bring it back.
Quick answer: Grab the missing bell from the red music box in the metal-cube room opposite the door, beat the boss it summons, then set the bells left to right: crowned human, mask, ripped chest, Configuration cube, Pinhead and followers.

Find the missing fifth bell
Once you interact with the bell puzzle, you will see four bells in place and one empty slot. The last bell is held in a room off the opposite side of the locked door, so head away from the puzzle to begin the detour.

Stock up on the ammo and crafting materials behind the gear before touching the music box. Those supplies are easy to carry into the fight the bell triggers.

After the boss is down, carry the bell back to the alley where the other four sit. Install the fifth bell in the empty slot, then set each bell to the symbol below, reading left to right across the row.

What happens after the door opens
You know the solution worked when the locked door swings open and a new path appears beyond the alley. A few mini-bosses stand between you and the next stretch. The following time you reach the Merchant, it marks the final boss encounter, so very little of the story remains after this point.
