The sequence begins with Quinzel escorting Joker into the office belonging to Warden Sharp. Amanda Waller identifies herself there and makes her pitch: she wants Joker on a team of villains. Sharp takes the gentler approach, dangling incentives to win him over, while Waller dictates the uncompromising conditions—Joker must leave for Belle Reve the following morning.

That order is what pushes Quinzel to speak up. She insists she can control Joker, which is her last attempt to keep her patient and her position at the asylum. Waller rejects the argument outright, and security guards physically throw Quinzel out into the hallway. The humiliation is the turning point. Sitting outside the office, she stops thinking of herself as his doctor and starts thinking of herself as his partner.

Nothing in the scene shows Joker asking her to do any of this. The switch is entirely her own decision, made after the asylum sides against her.
The costume store run and the Arkham break-in
Quinzel drives away from Arkham and stops at a novelty costume store, where she picks out a villain disguise off the rack. It is a deliberately unglamorous origin for the outfit. There is no lab accident and no chemical bath, just a woman buying a costume and driving back toward the asylum she was just removed from.

Back at Arkham, Joker is poking at the odds and ends scattered on his cell floor when the wall behind him detonates. Harley Quinn is on the other side of it, having used dynamite to open a hole big enough to walk him through. It is her first act under the new name, and it happens before the transfer to Belle Reve can go ahead.

She then hands Joker his classic purple suit. He changes on the spot and declares an ultimate going-away party, which closes the sequence with both characters fully in costume and back in business.

Across LEGO Batman, Harleen Quinzel is a psychiatrist interning at Arkham Asylum who is fascinated by studying The Joker. Their sessions turn into an infatuation, she breaks him out more than once, and her psychiatric license is revoked. She reinvents herself as Harley Quinn, calling him “Mister J” or “Puddin’” instead of by his title.
Two details carry over into the villain identity. She is a skilled gymnast, which is why her fights lean on acrobatics and a mallet rather than brute force, and she keeps her psychiatric training, which makes her unusually good at reading the people she works with. Her own surname is the joke: Harleen Quinzel sits one step away from “harlequin,” and the jester costume makes that explicit.
Poison Ivy is her closest ally outside Joker’s circle, and the two are consistently written as close friends in the LEGO continuity.

The pre-Harley identity has its own separate minifigure line, distinct from the Harley Quinn figures. It first showed up digitally in 2008 on portable versions of LEGO Batman: The Videogame, then took five years to reach plastic.

The 2013 figure is built around the reveal. One face has her in round black glasses with red lipstick, the other has sloppy Harley makeup that looks like it was smeared on in a hurry, and the light yellow hairpiece swaps out for a red and black jester’s hat found elsewhere in the set. Its back printing depicts the rear of her lab coat.
The LEGO Batman Movie version hides the same reveal in the clothing instead. Her white doctor’s coat runs down onto the legs with a red pen in the pocket, but the grey-and-red argyle sweater underneath uses the diamond pattern from the Harley costume, and the red cat-eye glasses match the shape of her Harley mask. LEGO’s own description for the figure leans into it, calling her a doctor certain to achieve “a great breakout – er, breakthrough,” with a Bat-Fact noting she looks familiar for some reason.
Dr. Harleen Quinzel also turns up in LEGO Batman: Legacy of the Dark Knight, which is where the Belle Reve confrontation and the costume store run belong. It is the fullest version of the origin LEGO has put on screen, and the only one that makes a prison transfer order, rather than a therapy session, the thing that finally creates Harley Quinn.
