Milly’s Harvest is one of the first collection quests a new Alliance character runs into, and it is deceptively simple. The grapes are not dropped by enemies. They sit in wooden tubs scattered across the Northshire Vineyards, and you click them like small chests while Defias Thugs loiter nearby.

Milly’s Harvest is the middle step of a three-part chain, and it does not appear until you have worked through the early Northshire quests. The path runs from A Threat Within into Brotherhood of Thieves, which is the quest that sends you east to collect Red Burlap Bandanas from Defias Thugs.
Turning in Brotherhood of Thieves to Deputy Willem in front of the Abbey opens two branches. One is Bounty on Garrick Padfoot. The other is Milly Osworth, a simple delivery quest that points you around the back of the Abbey to Milly herself and her wagon. Hand that in and she offers Milly’s Harvest immediately.

Pick up Bounty on Garrick Padfoot before you leave. Garrick is further southeast past the vineyards, so both objectives sit on the same trip.

Being hit mid-click interrupts the loot channel, which is the main reason people leave the vineyard short of eight. Kill first, loot second.
Turning in Milly’s Harvest and confirming completion
Run back northwest across the river to Milly Osworth’s wagon behind the Abbey and hand over the crates. Her completion dialogue thanks you for saving the harvest and mentions hoping you taught a few Defias a lesson — that text plus the experience and Stormwind reputation gain is your confirmation the quest closed out.

Milly follows up with Grape Manifest, the final link in her chain. That one sends you inside the Abbey and up the central stairwell to Brother Neals near the attic, which is also where the chain’s reward actually lands.
Which versions of WoW still have Milly’s Harvest
The quest was removed in patch 4.0.3a on November 23rd, 2010, when Cataclysm rebuilt the starting zones. In the modern game it is flagged obsolete and cannot be picked up or turned in, and the grape tub object is gone from the vineyard entirely.
It lives on in World of Warcraft Classic, where it was added back in patch 1.13.2 alongside the rest of the original Northshire chain. So if you are levelling a human through the Abbey on a Classic realm, Milly and her grapes are exactly where they were in 2004.
