The Acting Physician is one of the nastier Deviants roaming Nalcott, and it has a reputation for wasting ammo. Players kept dumping magazines into its body, watched it heal itself back up, and then got dragged into a grab. The fight only makes sense once you know that its housing is not the target.

Quick answer: Ignore the body. Wait for the glowing blue computer terminal to appear during its skill casts, destroy that, then kill the enemy normally once it stands back up.


Where the Acting Physician spawns in Once Human

This Deviant lives in medical spaces inside strongholds in the workshop area, so hospitals, medical rooms, and operating rooms are where you will run into it. Before it activates, it looks like a surgical or patient coat lying flat on the floor, with only faint static sparking around the edges. Get close and a harsh blue light fills the coat, orb lightning inflates it, and the thing stands up.

In its exposed form you can see the glowing terminal across its chest and abdomen, a medical headlamp on its head, and four mechanical tentacles spilling out from under the coat. The feet use suction cups, so it can stick itself to walls and ceilings mid-fight and drop on you from an angle you were not watching.

The nameplate above the enemy reads ACTING PSYCHICAN in game, even though it is known as the Acting Physician everywhere else. Same enemy.


The terminal weakspot rule: why body damage does not finish it

The terminal is the real creature. The coat and tentacles are a shell. If you push the Acting Physician to low HP without breaking the terminal, it triggers a recovery and claws its health back, which is exactly the loop that made players ask for a nerf in the first place.

The terminal is not always available. It is only exposed while the enemy is casting certain skills, so the fight is really about waiting for those windows and hitting them hard enough to matter.

Energy drain: eat before you fight

The Acting Physician is obsessed with your stomach, and that obsession is a real mechanic rather than flavor text. Its attacks go after your energy first, and what happens next depends entirely on how full you are.

  • Hits prioritize stealing your energy instead of only chipping HP.
  • A successful steal heals the Acting Physician.
  • If your energy hits 0, its attacks deal massive damage.
  • Getting grabbed drags you into its “surgery,” and a timely QTE struggle breaks you free.
  • Keeping food on hand and topping up mid-fight is the cheapest counter to all of this.

How to kill the Acting Physician

Show up with your energy bar full and spare food in your bag. Fighting this thing while hungry hands it free healing and turns its hits into burst damage.
Hold your fire on the body. Shooting the coat and legs burns ammo without moving the fight forward, so use that time to keep distance and stay out of tentacle range.
Watch for the blue terminal to light up while it casts. The moment it is visible, put everything into that panel. Butterfly makes a noticeable difference here and helps you clear the window in one pass.

Once the terminal breaks, the enemy collapses. Hold position for a few seconds and let it get back on its mechanical legs instead of panicking and repositioning into a corner.

Open up on it normally. With the terminal gone, the health bar goes down and stays down.

How to tell the terminal actually broke

You get two clear signals. First, the Acting Physician drops to the ground the instant the terminal is destroyed, which does not happen from regular body damage. Second, after it stands back up it stops grabbing and stops healing itself, so its health bar only moves in one direction.

If you push it low and it suddenly recovers HP instead, the terminal is still intact. Back off, stop feeding it energy, and wait for the next cast window.

The complaints landed. Feedback about how punishing this enemy is has been accepted, and a nerf is being planned. No specific patch, date, or list of changes has been confirmed yet, so there is nothing concrete to plan around beyond knowing adjustments are on the way.
