Every run in Pathogenic starts the same way. You pick a parasite, then fight your way through the human body one organ at a time, grafting organelles as you go. The parasite you choose shapes how fast you level up, how well you dodge, and how many upgrades you can stack before the final push toward the brain. Some choices smooth out the whole journey, while others ask for real practice before they pay off.

Quick answer: Pick Amoeba for the fastest leveling, or Helminth if you want a defensive parasite that can block incoming attacks. Both are the strongest starting picks for a clean run to the true ending.

S-tier: The strongest picks

The two S-tier parasites give you the smoothest path through the host. They reduce the friction that usually slows a run down, whether that friction is slow leveling or taking too much damage.

Amoeba needs 20% less DNA to level up. That gap looks small at first, but it compounds across a full run. You gather more upgrades earlier, hit power spikes sooner, and reach the advanced organs with a stronger build than most other parasites can manage in the same time.

Helminth is the defensive standout because it can block attacks outright. It cannot strafe, so the movement takes some getting used to, but its blocking makes up for that limitation. Once you learn the rhythm of its movement, it turns into a steady, balanced parasite that soaks up mistakes.

A-tier: Reliable but slower

A-tier parasites are close to the top picks, but they ask for a little more time and care to carry you to the brain. Both are still solid choices for most runs.

Bacterium is the first parasite you can play, and it is built around balance. It has no glaring weakness and no standout strength, which makes it a clean starting point for learning the game. There are no complex mechanics to juggle, so beginners can focus on movement, aiming, and grafting organelles.

Lil Collector hands you a 20% shop discount, so you can stockpile far more upgrades over a run. The trade-off is mobility. It can get stuck against walls, and while the dodge button pulls it free, you will spend the run steering carefully to avoid getting pinned.


B-tier: Niche and demanding

The B-tier parasites can perform well, but only under the right conditions. Each one carries a limitation that raises the skill floor or depends on a specific way of playing.

Fungal Spore gives you extra organelle slots, which is exactly what you want if you like packing many organelles onto one body. The catch is that it cannot rotate, so you lose a chunk of your ability to angle away from incoming damage. That defensive gap keeps it out of the higher tiers despite its build potential.

Diatom sits in B-tier purely because of how it plays. Two players control the same parasite at once, and you will need at least one controller for it. With a coordinated partner it climbs toward the top and delivers the best co-op experience in the game, but the shared control and hardware requirement hold it back for solo-minded players.

Nanobot: Not yet ranked

Nanobot is the armor-stacking parasite and the last one left to unlock on many save files. Until it has been unlocked and played through a full run, it stays off the tier list rather than getting a placement it hasn’t earned. Expect it to slot in once there is enough hands-on time to judge how its armor stacking holds up in the later organs.


If you are still learning the game, start with Bacterium to get comfortable, then move to Amoeba or Helminth once you want the smoothest path to the true ending. Save Fungal Spore and Diatom for when you want a specific challenge or a co-op session with a partner.
