Rusted metal doors show up early in Silent Hill: Townfall and they never respond to a key. Each one has a square hatch in the middle, sealed over with tape, and the only way through is to cut that tape open and reach inside for the latch. The tool that does it is the Scalpel, and you cannot get it until you have been inside the Shorefront Family Clinic.
Quick answer: Grab the Scalpel from the clinic, then interact with any rusted door with a taped hatch, select the Scalpel, slit the tape, and reach through to unlock it from the other side.
How to recognize a rusted metal door
These doors are flat metal panels covered in rust, usually blocking alleyways, shop rear entrances, and shortcut routes between blocks. The detail that matters is the square hatch taped shut at roughly chest height. If a locked door has that hatch, the Scalpel is the answer, and no code or key item will substitute. Doors without a hatch use something else entirely, so do not waste time on them.

The clinic shutter is sealed by an electronic card reader, so the Scalpel run really starts at Zoe’s House, west of the town square. The organ transport container sitting in the kitchen holds the badge you need, and it is locked by two separate three-digit dials.

The two codes come from Zoe’s email on the house computer and the Organ Transplant Card, so pick both up if you want the clues rather than the shortcut.
Scalpel location in the Shorefront Family Clinic
Cross the foggy town square, walk past the market stalls, and head for the clinic’s shuttered entrance. Everything from here is a short, linear detour.

Cutting open a rusted metal door

You will know it worked when the door swings open and you can step into the room or alley behind it. Opening your map from the new position confirms the route is now connected.
Rusted doors the Scalpel opens
| Rusted door | What it leads to |
|---|---|
| Pharmacy (7) | Back room with the rusty locker |
| St. Amelia’s Park, south side (11) | Northern part of the district |
| Alleyway near the town square | Storage room |
| Flemington Street | Story route in the north |
The park door is the important one for progression, since cutting it open is what gives you access to the northern stretch of the district and the objectives waiting there. Once the Scalpel is in your inventory, every rusted door you come across in the Shorefront District works the same way, so it is worth cutting each one open as you pass to keep routes between blocks short.
