The Sanguine Caverns sit in the northernmost stretch of Fainweald, a bleak expanse of white stone streaked with pools of blood. You can reach the entrance well before the game formally pushes you there by running a straight overland path from the Widow’s Overlook beacon through Blackridge Pass and Blackridge Cliffs, then dropping onto a lower path that leads to the cave.
Quick answer: From the Widow’s Overlook beacon, head through the stone arches, cross the wooden bridge to the Blackridge Pass beacon, follow the canyon into Blackridge Cliffs, drop off the cliff edge marked on your map to the grassy path below, then cross the final wooden bridge into the snowy cave entrance.

The whole run takes only a couple of minutes if you skip most fights. You can bypass wandering enemies rather than clearing them, since the goal is the cave entrance, not the loot along the way.

How to confirm you’ve reached the Sanguine Caverns
You know you have arrived once you step off the wooden bridge into the snowy cave mouth. The area shifts to the caverns’ signature look, with white stone walls and streaks of blood, and small enemies plus another bone-spider will engage you almost immediately inside.
Note: There is a separate overworld ring near the entrance with two large enemies and a pair of tents. Ventrium appears here as common loot, and you can grab a Tooth of Nochte from a statue on the left as you head up the ramp, plus a Synaptic Vessel from crates near a lit campfire once you move inside.
Where the caverns lead next
Past the campfire, a dark alley runs to the Stonebled Gate beacon, home to the dungeon The Hidden Keep. Cleansing that beacon opens the gate into the deeper Sanguine Caverns region and rewards the Serpent Stone. The area also connects outward, with a southeastern bridge that leads to the Stones of Remembrance and a northern bridge in Gloomshade Grove that brings you back into the caverns, so you have more than one way in once the map opens up.
