The Arbiter’s Medallion is one of the stamina-focused legendary amulets in The Blood of Dawnwalker, and it does not come from a quest, a merchant, or a boss. It sits on a corpse in an unmarked cave, and the only thing standing between you and it is a waterfall you have to walk through.
Quick answer: Head to the river bend in Boar’s Back north of St. Toya’s Grave, move behind the double waterfall into the hidden cave, and loot the skeleton lying on the cave floor.

The cave has no icon on the map, so you are aiming at a stretch of river rather than a labelled point of interest. Open the world map and place a custom marker on the river bend directly north of St. Toya’s Grave, inside the Boar’s Back region. The same spot reads as southeast of Parlita and northwest of the Sandy Gorge shrine, so any of those three landmarks works as your anchor.

Sandy Gorge is the most convenient starting point if you have already unlocked that shrine, since the run from there is a straight descent into the canyon. Parlita works too, just with a longer approach over ground.

This is a Level 20 legendary amulet built almost entirely around stamina economy. It raises your maximum stamina pool while cutting what you spend on attacks and blocks, with a small slice of physical damage reduction on top. The exact percentages roll rather than being fixed, so your numbers may land higher or lower than another player’s.

Note: the variable slot can roll negative. A weapon damage penalty on the same amulet is possible, so check the tooltip before you commit to it over another accessory.
If the medallion is not in your inventory
You will know the pickup worked because the amulet appears under accessories in your inventory with the legendary tier label and a Level 20 tag. If it is missing, there are only a couple of likely reasons.
- You stopped at the wrong waterfall. The correct spot has two falls side by side, with fallen trees in the shallow water in front of them.
- You entered the cave but walked past the body. The interior is very dark, and the skeleton lies flat on the ground rather than propped against a wall, so use the loot highlight before you leave.
- You looted the corpse but only took part of the stack. Confirm the amulet itself was transferred, not just the trousers and coin.
Because the cave is free-roam loot rather than a quest reward, nothing gates it behind story progress, and there is no timer pressure to grab it early. It is also one of the pieces that counts toward filling out a full legendary loadout, which makes the short detour into the canyon worth taking whenever you next pass through Boar’s Back.
