Part 3 of Silent Hill: Townfall drops Zoe inside St. Amelia Medical Center with a shopping list of medications, and one of them is sealed behind the Records Room door. The door has no handle puzzle and no key. It has a keypad, and the keypad wants four digits you have not picked up yet.

Quick answer: The note with the four-digit PIN sits on the board to the left of the computer in the upstairs Office, in the northwest corner near the Private Ward. Enter it on the keypad to get inside.

Why the Records Room matters in Part 3: Patient

The chapter tasks you with tracking down three medications inside the medical center: Hydroxalene, Tensobromide, and Dextracylate. You cannot finish the section’s dosage puzzle without all three, and the game will not let you skip ahead even if you already know where everything is.

The Records Room holds 3mg of Hydroxalene. That makes the locked door a hard gate rather than an optional detour, so the keypad has to be dealt with before the chapter moves on.

How to reach the upstairs Office and find the note

Pick up the St. Amelia Medical Center map at the start of the chapter. It is on the wall to the left of the Operating Room, and it labels every room name you need, including the Records Room and the Private Ward.
Make your way to Patient Room A and take the staircase directly to its left. That staircase is the only route to the upper floor of the building.
Upstairs, walk to the northwest corner and step into the Office next to the Private Ward. The note is pinned to a board on the left side of the desk computer, and examining it adds the PIN to your records so you do not have to memorize it.

Keep your flashlight up while you cross the upper floor. The Office board reads as a dark rectangle from the doorway, and it is easy to walk past the note entirely.

Opening the door and collecting the Hydroxalene

Head back down to the Records Room and interact with the electronic keypad mounted next to the doors. The panel has buttons 1 through 0 plus X, Y, and Z, but only the digits matter here.
Punch in the four digits from the Office note. Each button lights up as you press it, which is your cue that the input is registering.

Once the fourth digit goes in, the doors swing open on their own. Step through into the room, which is lined with metal filing cabinets and storage shelving.
Examine the metal cabinet on the left-hand side. That is where the 3mg of Hydroxalene is stored, and picking it up crosses one of the three medications off your list.

You will know it worked when the doors open without a rejection tone and the Hydroxalene appears in your inventory. If the keypad does nothing, you have not read the Office note yet.
