The three weighing scales in the Shorefront Family Clinic are the wall that ends Part 1 of Silent Hill: Townfall. Each platform takes one specific key item, and the game will not accept a partial arrangement — you need all three in hand before anything happens. Two of them sit on the other side of town, behind a scalpel and a locker code.

Quick answer: Put the Broken Stethoscope on the left scale, the Prescription Dosage Error Note in the middle, and Zoe’s Lanyard on the right.

The logic comes from the Resignation Letter found in the clinic. Its three lines map onto the three platforms in order.

  • “I didn’t listen closely enough” — a stethoscope is a listening tool, so it goes left.
  • The statement about making errors and losing focus identifies the dosage error record; place it in the middle because it represents the mistake.
  • The clue says the speaker can no longer work in the role. Zoe’s Lanyard represents that role, so place it on the right.

What you need before touching the scales

Zoe’s Lanyard is the one item you almost certainly already have, since it unlocks the clinic itself. It comes from the organ transport container in the kitchen of Zoe’s House, west of the town square, after you restore power and read Zoe’s email. The container dials are 956 on the left and 133 on the right.

Inside the clinic, the keypad on the reception door takes the code 2896, and the desk beyond it holds the Nurse’s Rounds Form and an answering machine message pointing Nurse Ellis toward the pharmacy on Main Street. The single most important pickup is the Scalpel, sitting in a green medical case on the desk in Z Ellis’ office at the end of the corridor. It opens every rusted door with a hatch in the middle, including both doors you need next.

Without the Scalpel you cannot reach either missing item, since both the pharmacy and St. Amelia Park are sealed behind hatch doors.


Prescription Dosage Error Note location (pharmacy, code 6539)

Leave the clinic and head north through the fog to St. Amelia Pharmacy. Interact with the sealed door and select the Scalpel to cut through the panel covering the latch.
Sweep the front counter area for the Restock Issues notice and a First Aid Kit on the shelves behind the till. Neither is required for the scales, but the kit is worth grabbing before the streets get busier.
Tune the CRTV on the desk to channel 30 and line up the screen borders and RGB calibration bars until the transmission resolves. The broadcast establishes the order of the posters hanging around the pharmacy, which is where the locker combination comes from.
Go to the rusted locker in the back room and enter 6539. Take the Prescription Dosage Error Note from inside.

Broken Stethoscope location (St. Amelia Park route to Jess’s House)

Walk to the south side of St. Amelia Park and use the Scalpel on the door panel there. This is the only way into the residential streets where Jess lives.
Follow the alleys past the park. Strange Postcard 2 is on the ground beside a red crate just inside, and Strange Postcard 3 is stuck to an exterior wall further along the street. They are collectibles rather than puzzle items, so pick them up as you pass.

Continue down the street toward building 14. A creature patrols this stretch, and a wooden plank is enough to put it down if you would rather not burn resources.
Enter the apartment block opposite and go upstairs to apartment 1/2. The 21 Farrow St. – Back Door Key is sitting on the dining table. The Feeling Sick Note is on the bedside desk in the bedroom, and Strange Postcard 4 is on the floor of the next bedroom along.
Head back out and find Jess’s House next to W.M. Clark Butchers, near building 14. Jess’s Door Note is pinned to the front entrance; the way in is the back door up the exterior stairs in the alleyway, which the 21 Farrow St. key unlocks.
In the bedroom, take the Broken Stethoscope from the open medical bag on the bed. While you are inside, the kitchen drawer holds the Lock Box Key, which opens the blue lockbox in apartment 071 for an Empty IV Bag and IV Fluid. Combine those two in the inventory to make a Filled IV Bag before you head back.

Placing the items and confirming the puzzle is solved

Retrace your route through the park gate and back into the Shorefront Family Clinic. The three scales are in the back room at the end of the corridor, the same area marked for Z Ellis. Interact with them and assign each item to its platform: stethoscope left, dosage error note middle, lanyard right.

You will know it worked the moment the third item lands. There is no confirmation prompt to hunt for — the game rolls straight into a cutscene, the Resignation trophy pops, and Part 2 begins. The order cannot be brute forced item by item either, since the game only checks the arrangement once all three platforms are filled, so gather everything first and then make a single pass at the scales.
