Lair Keys are the material you spend to open the Hoard chest that appears after an Initiate Lair Boss dies in Diablo 4. Without a key, you can still fight bosses like Grigoire, Varshan, Lord Zir, the Beast in the Ice, and Urivar, but you cannot claim their targeted loot. In Season 15 the fastest way to build a stockpile is a specific Helltide setup, backed by a handful of activities that drop keys as you play.
Quick answer: Run a Helltide with the Writhe and Rot node active on the Helltide War Plan, then kill the Pangs of Duriel that spawn from Hellwyrms and open Tortured Gifts. Both drop Lair Keys.

What Lair Keys open and what you need first
For any Initiate Lair Boss, one Lair Key is enough to unlock its Hoard. Summoning requires Torment I difficulty or higher. The boss encounter itself can begin without a key; the key is consumed for the post-fight chest, meaning each group member who wants loot needs to supply one.
Keys drop from several activities across Sanctuary, including War Plans, Helltides, World Bosses, Legion Events, the Tree of Whispers, and Kurast Undercity. Running more than one of these at once keeps your supply topped up while you chase uniques.

The Writhe and Rot node sits in the central chain of the Helltide War Plan. It changes how Hellwyrms behave inside the activity, turning a normal Helltide into a dedicated key farm. This is currently the most consistent way to target Lair Keys in Season 15, and it doubles as one of the best experience farms in the game.


Adding the third node, Bursting Blood, lets Maggots and Pangs of Duriel award Boss Trophies too, so the same route can supply materials for the bosses you are targeting.
Backup sources for Lair Keys
If a Helltide is not active or you want to layer activities, several other systems drop Lair Keys. None of these match the Helltide route for raw speed, but they cost little extra effort when you are already running them.

War Plans are worth prioritizing among these because they feed the others. Completing a full run can hand you Tribute of Titans caches and Escalation Sigils, which unlock the Kurast Undercity and Escalating Nightmare Dungeon runs that produce keys of their own.
Tributes of Titans cannot be purchased and only come from War Plan caches, so when you finally get one, bring your fastest build to make the most of the shortened Undercity timer.

Greater and Superior Lair Keys
Do not confuse the base Lair Key with its higher tiers, which open tougher bosses. Each tier comes from a different place in the progression chain.

After you defeat an Initiate Lair Boss and open its Hoard with a Lair Key, you receive a Greater Lair Key, which is the currency for the stronger bosses above. Initiate bosses can also drop a Stygian Stone that converts into any Greater key one-for-one.

Superior Lair Keys are the rarest of the three. They come from Belial ambushes, and defeating high-Torment bosses raises how often that mini-Belial appears after a fight. Blizzard also points to the Deathtoll Chamber as the best source for Superior Lair Keys at Torment IV.

You can reach the Deathtoll Chamber through Realmwalker events or a Forgotten Depths Nightmare Sigil carrying the Ruptures affix. When a mini-Belial ambushes you after a boss kill there, defeating him drops a Superior Lair Key on the floor along with a second round of the boss’s loot.

Confirming your keys are working
Check your inventory after each Helltide. Collected Lair Keys sit in your materials tab, and their count rises as you kill Pangs of Duriel and open Tortured Gifts. When you reach a Lair Boss, the Hoard chest prompt will read that it requires one Lair Key, and interacting with it consumes the key and opens the chest. If the chest opens and drops loot, the key was accepted and your farm is paying off.
