Skip the Emperor to lock in the Witcher path
Once the battle at Kaer Morhen is over, Geralt has the option of bringing Ciri to Vizima to meet her father, Emperor Emhyr var Emreis, on the way to Velen. Whether to make the visit is up to you; this choice determines whether the Witcher or Empress ending is available.
For the Witcher outcome specifically, do not visit Emhyr at all. Going straight to Velen removes the imperial route entirely, so a surviving Ciri chooses her own life rather than the throne.

How the hidden score works
Ciri returns from the White Frost when positive marks are equal to or greater than negative ones. If you also visit Emhyr and trigger the fifth mark, you need three positives.
To be safe, aim for all five positive choices and skip the Emperor. That combination clears the survival threshold and keeps her off the imperial track, which is exactly what the Witcher ending needs. If negative choices outnumber positive ones, Ciri does not come back, and the game plays its bleak conclusion instead.
How to know it worked
The result plays out during the epilogue quest “Something Ends, Something Begins.” Geralt informs Emhyr that Ciri gave her life in the last battle to stop the White Frost. Later, he reaches White Orchard’s old garrison outpost, where a blacksmith he hired has made a special silver sword.
Back at the White Orchard inn, Geralt meets Ciri again and hands her that silver sword. That moment confirms she is now a full Witcher, ready to set out on the Path. If you see this reunion and the sword handoff, you have the ending you were after.
