The gland is a misc hunting trophy that drops from rare Minecrawler specimens. You are guaranteed to pick one up during the Minecrawler’s Nest quest in Chapter 2, so there is no need to grind for it earlier. A small number of glands also lie out in the open at fixed spots in the world, which means you can stumble onto one even outside the quest.

Cavalorn the hunter introduces the gland through his Beasts of Khorinis questline. After you bring him the second Bestiary of Khorinis book, he explains that some Minecrawlers carry special pheromone glands and asks you to bring one if you ever find it. Completing that book delivery rewards you with 500 XP and sets up the trade.
When you later show up with the Queen’s Pheromone Gland, you can choose to sell it to Cavalorn in exchange for knowledge. That trade teaches the Extract Organ skill, which lets you harvest the poison sac of Bloodflies and the venom glands of Fire Lizards. Those organs convert into ore quickly, so the skill is genuinely useful for building up a reserve of Ore Nuggets.

Keep it or sell it to Cavalorn
The trade is tempting, but the recommended play is to hold onto the gland. It can be needed in a later chapter as an ingredient for crafting a Permanent Potion, and a permanent stat boost is worth more than the convenience of learning Extract Organ early. If you part with the gland too soon, you lose access to that recipe.

If you have no interest in permanent buffs and want faster ore farming, trading it to Cavalorn for Extract Organ is a reasonable alternative, but you cannot have both.
