The Second Floor update opens a whole new level above the wards in Animal Hospital, and most of it exists to support one system: pulling DNA out of your patients and turning it into experimental vials. There is also a break room, a hidden hatch, and a piece of lore that hints at rooms the hospital sealed off years ago.
Quick answer: Once you have had your first ambulance visit (or you started the run with Fast Start), head to the stairs marked “UPPER FLOOR” around Aisles 6 to 8, grab the DNA Sampler in the Lab, sample a patient, then run the sample through the Sequencer and the Vial Press to get a finished vial.

How to unlock the second floor and where the stairs are
The upper floor is gated behind your first ambulance visit. If you skip the tutorial with Fast Start when you deploy, the floor is available from the opening shift instead, which is the quickest route if you only want to test the Lab. The experience page on Roblox is where you launch into a session.
The stairwell sits in the corridors around Aisles 6 to 8 and is signposted “UPPER FLOOR” above the door. Walk up to the handle and take the on-screen prompt to open it, then climb. There is no key or code involved.

Two rooms wait at the top. One is a staff break room with a “Run Fast” vending machine and a bulletin board of sticky notes, including complaints about a new intern with smelly feet and a countdown reading “4 Days before I can go home…” The other is the Lab, and that is where everything mechanical happens.

The Lab holds three pieces of equipment you need to learn. The DNA Sampler is the handheld tool you carry down to patients, the Sequencer grows a sample into a culture, and the Vial Press turns that finished culture into a usable vial. Around the walls are ingredient shelves labelled with the four DNA letters, and each letter dispenses a different feed for the culture.

If the culture goes unstable
An unstable chain cannot be salvaged mid-process. You have to start the whole sequence again from a fresh sample, which means another trip down to a patient with the sampler. Keep track of the letter order you fed in so you are not repeating a combination that already failed.
What DNA vials do to patients
Finished vials are used on patients, and the result depends on the serum you produced. Effects include shrinking a patient down to a tiny size, making hair sprout on them, and knocking them out cold. Since fainted patients create work for you, be deliberate about who you test an unknown serum on during a busy shift.
The hidden hatch and the clown jail
One corner of the Lab floor has a hatch built into it. Drop through and you land in a small jail holding a clown-like NPC. Talk to him and he rambles about balloons, including a claim about spending eighteen years with balloons and the Laboratory.
He eventually vanishes and leaves an Animal News item behind. Picking it up completes the “Clown” discovery, so that is your confirmation the secret registered. The newspaper reports that the Laboratory and several patient rooms were closed and sealed with no reopening date given, alongside a story about a fatal collision between a passenger car and a Nom Aly Burgers delivery truck.
Note: the room has a second exit that comes out at the new locker in the first-floor receptionist room. Once you know it is there, you can move between reception and the Lab without using the stairs at all, which saves a lot of time when a shift gets hectic.

The DNA system is the part worth spending real time on, because the combinations are something you work out yourself rather than an item you simply collect. And the newspaper’s mention of wards that were sealed rather than demolished leaves an obvious door open for whatever gets unlocked next.
