Animal Hospital fills its night shift with animal faces, and figuring out who actually wants to help you is half the job. Some of them keep the hospital running, one of them will kill you if you say the wrong thing, and a few just sit in the lobby. Knowing which is which changes how you play every shift.

Quick answer: Dr. Harlow, Ratthew, Ron from Accounting, and the Shopkeeper Nurse are staff or allies, while Officer Duckman reacts to your choices. Barney is the one dangerous character, a serial killer who threatens you directly, so treat him with caution.


All Animal Hospital characters and their roles

There are nine named characters you can run into across the hospital, from the reception desk to the lobby. Here is each one and what they bring to your shift.

Dr. Harlow, the doctor who runs your shift

Dr. Harlow appears in every shift and is the closest thing you have to a guide. He is a light orange, deer-like figure in blue scrubs under a white coat, with a stethoscope, a head mirror, and a black “TOP SECRET” suitcase. He shows up at the end of each shift to grade how you did.

His real value is what he hands you over time. After Shift 1 he gives advice and unlocks the shutters and cameras. In Shift 2 he opens the shop and the emergency rooms (Rooms 6 through 8). In Shift 3 he unlocks the taser. There is also a quiet hint that he may be more than he seems, since an anomaly version of him can be spotted in one of the lobby windows.


Barney, the serial killer you have to handle carefully

Barney debuts in Shift 3 and is the one character you should genuinely fear. He looks like a friendly white rabbit with black slit eyes, a mustache, a brown hat, and plaid pants, but his anomaly form swaps that for a wide, sharp-toothed mouth and big hollow eyes. He is an anomaly and a serial killer who uses a kitchen knife, and he has already used it on Officer Duckman.

With the police closing in, Barney asks you to hide him in the office. He is polite at first, calling you “friend” and thanking you for help, but that turns fast. If he thinks you might report him, he warns that “friends do not turn each other to the police” and tells you he will kill you if you speak.

Hiding Barney leads to two outcomes. Turn him in and he runs off after knocking out Officer Duckman. Keep him hidden and he thanks you, then hands over his suitcase, which holds a second coffee machine. You can also reject him outright by closing the shutters or letting his timer run out.


Officer Duckman, Ratthew, and Ron from Accounting

Officer Duckman is the police officer tracking Barney, and his story is tied directly to your decisions. If you turn Barney in, Duckman makes the arrest. If you keep quiet, he leaves without incident. Whatever happens here is on you.

Ratthew is kinder than he looks. He is cautious and hides who he really is when you first meet him, mostly because past experiences left him distrustful. Stick with him and the friendlier side comes through.

Ron from Accounting is your blunt source of information. He understands the anomalies and monsters in the hospital and tells you about them plainly, without trying to make the threats sound less serious than they are.


The Shopkeeper Nurse and lobby characters

The Shopkeeper Nurse staffs the shop, which becomes available once it opens in Shift 3. That is where you spend coins between shifts on the items that keep you alive.

The rest are atmosphere. The Lobby Cat and Lobby Bunny hang around the hospital lobby, and the News Bunny delivers news within the setting. They flesh out the world without changing how a shift plays out.

The split is simple once you know it. Lean on Dr. Harlow, Ratthew, Ron, and the Shopkeeper Nurse, weigh your choice carefully with Officer Duckman, and stay sharp around Barney. New faces tend to show up as the game updates, so the lineup may grow over time.
