Vasto Lorde is one of the newest Ichigo-themed weapons in Lineage Piece, and it lives entirely on Hueco Mundo. Unlocking it means finishing a short questline handed out by the Kisuke NPC, then repeatedly beating the Vizard Ichigo boss. You will also need Tensa Zangetsu equipped before you start, since two of the quest steps only count when that weapon is doing the work.
Quick answer: Talk to Kisuke at Inner Hueco Mundo, complete all four of his quests using Tensa Zangetsu, then summon and defeat Vizard Ichigo 15 times on Hard or Extreme to unlock Vasto Lorde.
What you need before starting
The questline assumes you already own Tensa Zangetsu, because the second and third quests specifically track kills and skill uses with that sword. If you have not picked it up yet, grab it from the Ichigo NPC on Hueco Mundo first. You also need a steady supply of Tainted Soul Pills, which fuel the boss summon later in the chain.

Find Kisuke at Inner Hueco Mundo
Open the teleport pad and select Hueco Mundo. Fly toward the large white building in the area labeled Inner Hueco Mundo, where the Kisuke NPC waits with a Talk (E) prompt. Speaking to him starts the four-part questline that leads to Vasto Lorde.
Complete the four Kisuke quests
The quests must be done in order, and each one pays out the same reward of 17K Coins and 1.7K Gems on completion. Here is what each step asks for.

Summon and defeat Vizard Ichigo
Vizard Ichigo does not spawn on his own. You bring him out at the summon pedestal on Hueco Mundo, and each summon consumes a set of materials. Selecting the Extreme difficulty is the fastest route, since the questline accepts Hard or Extreme clears.

Because you need 15 clears, plan on stockpiling Tainted Soul Pills and Summon Cores in advance. Beating the Ichigo Bankai boss between summons refills both, along with extra rewards like Trait Rerolls and Reroll Essence.
How to know Vasto Lorde is unlocked
Once all four quests read as complete, Vasto Lorde becomes available and appears in your weapon inventory alongside your other swords. If the fourth quest is stuck, confirm your Vizard Ichigo runs were on Hard or Extreme, since Easy and Medium clears do not count toward the 15 required kills. Returning to Kisuke after the final clear finishes the chain.
With Vasto Lorde in hand, you have one of the stronger Ichigo weapons on Hueco Mundo, and the same island trips you took for this questline also feed into the other Bleach content added in the update.
