The Hollow Core is a rare item in Lineage Piece that only starts dropping once you clear a five-stage questline on Hueco Mundo Island. Until that questline is finished, no enemy will drop it, no matter how long you farm.

Where the Hollow Core Questline starts
Everything begins on Hueco Mundo Island. Fly to the top of the tower there and interact with the Hollow Core Questline NPC. This NPC hands you the objectives one at a time, so you cannot skip ahead or work on them out of sequence.

The first three stages are straightforward mob farming around Hueco Mundo. The last two stages shift to bosses, which is where the summon system comes in.

Summoning the bosses for stages 4 and 5
The final Aizen stage requires you to summon the boss yourself using the Summoner NPC. That NPC offers several summon options, so pick the one the stage calls for.

Defeat the Ichigo (Darko) boss 25 times to clear stage four, then summon and beat Aizen five times to close out the questline.
How the Hollow Core actually drops
Finishing the questline does not hand you a Hollow Core directly. Instead, it flips on the drop. Once all five stages are complete, any NPC you defeat has a small chance to drop a Hollow Core, which then appears in your inventory.
Because the drop rate is low, you may need to keep farming mobs and bosses after the questline before one appears. There is no guaranteed drop, so repeat kills are the only way to secure it.

How to confirm it worked
You know the questline is done when the Hollow Core Questline NPC no longer offers a new objective, and all five kill counters are complete. From that point, watch your inventory after each kill. The Hollow Core shows up there as a named item once it drops.
If nothing drops, the most common reason is simply the low drop chance. Keep defeating enemies and the item will eventually appear, as long as every stage of the questline is finished first.
