Escape is the 13th mission of the Modern Warfare 4 campaign. It puts you in the boots of Captain John Price as he breaks out of a North Korean prison, and it hides one optional collectible: a set of dog tags locked inside a wall safe. Opening that safe unlocks the Never Forgotten trophy. The code is written on a note early in the level, but the safe itself sits much later in the mission. If you don’t grab the note on the way through, you won’t have the code when you reach the safe.
Quick answer: After the barracks fight, take the first door on the right into the Security Room and enter 8410 on the wall safe. The dog tags drop immediately and Never Forgotten unlocks.

Never Forgotten trophy requirements in Escape

The trophy is the only payoff. Recovering the dog tags doesn’t grant any in-game reward or cosmetic, and nothing about the rest of the mission changes once you have them.

The route runs in two halves. You first get the code during the stealth opening, then you reach the safe during the firefight with Ghost. Story spoilers for Escape follow.
Get safe code 8410 from the Updated Code note
Open the wall safe after the barracks
If you’ve read both of the mission’s notes, the keypad prompt shows “Notes Found 2/2” along with the code. That’s a quick way to confirm you picked up the Updated Code note before you start typing.

How to confirm Never Forgotten unlocked
Never Forgotten pops the moment you collect the dog tags from the safe. You don’t have to finish the mission first. After that, Escape has nothing else hidden in it. You can head up to the garage control room and finish the level as normal.
If you already played past the Security Room without opening the safe, you haven’t lost the trophy. Escape can be replayed at any time, and the trophy works on any difficulty. Load the mission again, grab the Updated Code note during the stealth section, and follow the same route back to the safe.
