Onimusha: Way of the Sword hides nine Hozuki Pouch materials across Kyoto. These are the four Oni Cloths (Regeneration, Aggressor, Resistance, and Awakening) plus five Skeleton Hozuki. Collecting all of them, then fully upgrading each pouch, unlocks the Medical Marvel trophy and achievement.
Quick answer: Grab all 9 pouch items from the Southeast, Southwest, Shijo Street, and Yasaka Pagoda areas of Kyoto, then use Okuni’s crafting menu (unlocked in “Patching up the Pouch”) to enhance every pouch to its max level. Each Level 3 upgrade consumes one Skeleton Hozuki.
When you can start collecting and the point of no return
Pouch upgrading first becomes available when you speak with Okuni during the mission “Patching up the Pouch.” She sends you out for the materials needed to bring the pouch to its first upgrade level. From there, you can freely collect every item below.
None of these items are missable during normal play, but the final mission locks the map. You can free-roam and clean up all collectibles right up to the Point of No Return before “The Clash at Rendaino.” Keep a manual save at that point, and you can return later to gather anything left. After finishing the story, selecting Continue from the main menu also drops you back before the finale with progress intact.
Tip: Buy Oni Vision: Treasures, Oni Vision: Treasures 2, and Oni Vision: Search Range at the Spirit Mirror before exploring. Hold L2/LT and tap R2/RT to reveal hidden chests through walls, which makes several of these Oni Spot chests far easier to find.

Southeast Kyoto: Oni Cloth (Regeneration) and Skeleton Hozuki #1
Both chests sit next to each other just inside an Oni Gate in the southeastern part of the city. You cannot reach them from the street.
Southwest Kyoto: Oni Cloth (Aggressor) and Skeleton Hozuki #2
This pair is in the north-central/southwest district near a watchtower and a barn. Two red barrier cores block the gate.


South of Shijo Street: Oni Cloth (Resistance) and Skeleton Hozuki #3
Both chests share a courtyard reached from the rooftops, blocked by a Malice Barrier. The area is near the docks in the northwest part of the map.

Yasaka Pagoda area: Oni Cloth (Awakening) and Skeleton Hozuki #4
This pair is guarded by the boss Byakko – Hundred Defilements. You need to clear a courtyard fight to reach both chests. Note that the Awakening pouch itself can only be enhanced after you defeat Shuten Doji later in the game.


Skeleton Hozuki #5: The “Hidden Treasure No. 5” side quest
The final Skeleton Hozuki comes from a Chance Encounter rather than a free-roam chest. You start it in the northeastern district.
Hozuki Pouch upgrade materials for Medical Marvel
Finding the items is only half of Medical Marvel. You also need to enhance every pouch to its maximum level. Each pouch reaches Level 3, and each Level 3 tier consumes one Skeleton Hozuki, which is exactly why you need all five. Fine Silk Cloth and Tamahagane also feed into these upgrades and are collected separately.

Once every pouch above sits at its top level, the Medical Marvel trophy and achievement pop. If it does not unlock, the usual reason is a pouch still sitting below Level 3, most often the Awakening pouch, since it only becomes upgradeable after Shuten Doji is defeated. Double-check that all five Skeleton Hozuki were spent, as each one is required for a different Level 3 tier.
