The Hallowed Concepts expansion adds 18 new skins to Risk of Rain 2, one for nearly every Survivor in the roster. Operator and Drifter are the only characters left out. Each skin is tied to a specific challenge, and you must complete that challenge while playing as the Survivor whose skin you want.
Quick answer: The 18 skins split into three groups of six. One group clears Soulbound Threshold before the fourth Teleporter, one defeats the hidden boss behind the Reincarnation door, and one finishes a run holding at least 10 Hexes.

Which Survivor belongs to which challenge
Find your Survivor in the table to see which challenge unlocks their skin. The skin only registers for the character you are controlling when the condition is met, so clearing a challenge as Huntress will not unlock Commando’s version.

To earn every skin, complete each group’s objective separately on all six of its Survivors. Enforcer and Acolyte have additional Mastery appearances, which are not part of the 18 challenge skins covered here.

This challenge covers Commando, Huntress, Acrid, Seeker, CHEF, and Enforcer. The deadline applies to finishing the encounter, not just opening its portal, so you need enough damage and survivability early in the run.

Soulbound Threshold is a boss rush, so arrive with boss damage, strong movement, and enough recovery to survive several fights back to back. Stage Two leaves more room before the deadline, while Stage Three gives you an extra stage of items at the cost of a tighter window.
Defeat the boss beyond the Reincarnation door
This challenge covers Bandit, Engineer, Artificer, REX, Captain, and Acolyte. It uses the same Soulbound Threshold entrance, but there is no fourth-Teleporter deadline, so you can keep collecting items until your build is ready for a second, harder fight.
The one item you cannot skip is the Reincarnation N’tormat Page. Pages come from Trials and from Shrines of Compulsion, but completing a Trial does not guarantee Reincarnation appears among the choices. Check your collected Pages before committing to the encounter.

The hidden fight is harder than the standard Soulbound Threshold boss rush, and you must clear the normal rush first. Bring reliable single-target damage plus enough movement and healing to keep fighting after the first set of bosses.
For Acolyte, unlock the Survivor first by performing the standard rescue. Then begin a new run as Acolyte and complete this encounter. Rescuing her while playing a different Survivor will not fulfill the condition for her skin.
Finish a run holding at least 10 Hexes
This challenge applies to MUL-T, Mercenary, Loader, Railgunner, Void Fiend, and False Son; Soulbound Threshold is excluded. Complete the run while carrying at least 10 Hexes. You can carry additional Hexes, but they incur penalties and contribute nothing beyond the required total.
There are two ways to gather Hexes during a run:
- Hallowed Shrines appear after Teleporter events. Choose Embrace Death to take three Hexes.
- Concept Reliquaries, the interactables shaped like small grasping hands, drop Hexes during normal stage exploration. A Sale Star can double the amount you receive.
Carrying the N’tormat makes Hallowed Shrine appearances more dependable. After every pickup, check the Hexes in your inventory instead of tallying the shrines you’ve activated. Starting with none, three Embrace Death choices give you just nine, so collect at least one more before finishing the run.
You can complete the run through any ending: defeat Mithrix and escape, loop before obliterating, or enter a DLC ending. Your Hex total is checked when the run ends, so keep it at ten or higher until then. Embrace Life at a Hallowed Shrine exchanges Hexes for rewards, and using it too late can leave you below the required amount.
Hexes are detrimental items that make the run harder, and their effects stack, cutting survivability, hiding parts of the screen, or disrupting abilities. Build your damage and recovery first, then stack toward ten on stages you can still clear.

How to confirm a skin unlocked
For both Soulbound Threshold challenges, a completion notification appears when you return to your run after leaving the realm. For the Hex challenge, the relevant skin becomes available once the run finishes with the count met. If no notification shows after a Soulbound Threshold clear, check two things before trying again: how many Teleporter events you had completed, and which Survivor you were playing. The early-clear window cannot be restored once the fourth Teleporter is done, and each skin names the exact character that must be used.
