How to confirm the Vampire Talisman is working
Open the Character Info screen and check that the Vampire Talisman is listed among your equipped accessories alongside the rest of your gear. If it is still showing in the bag instead, it is not active.

The in-combat proof is simpler. Attack a pack of mobs and watch your health bar instead of the damage numbers — with lifesteal running, your HP ticks back up with every connected hit rather than only draining. On an aggressive multi-target swing you can end an exchange at higher health than you started it.

Note: the healing is a percentage of damage dealt, so a whiffed swing returns nothing. Landing the highlighted Critical (R) prompt and keeping your combo going is what turns the talisman into a real survivability gain.

Surviving long enough to farm the Pale Beyond
Two systems can end a chest run before the drop ever appears. Sanity drains at night, and hitting zero kills you, so run your loops during the day where you can and keep a route back to a campfire or town to top the meter back up. Sanity potions serve the same purpose when you are too deep to turn around.
The other risk is other players. PvP only toggles inside the spawn safe zone, and once you leave town anyone can attack you regardless of your setting. Outlaws deliberately hunt prestige-carrying players, and dying costs a prestige level along with the 50 HP that came with it — a real setback when you are grinding the same chest route for hours.
Once the talisman is equipped, its value compounds with everything else you are building. Higher weapon damage, enchants from the blacksmith, and prestige HP all raise the ceiling on how much each hit returns, which makes the Pale Beyond grind pay for itself on every run after it.
