The Door with Indents sits in the Central Hall of the Rhodes Hill Care Center, and it has three empty slots labeled Moon, Sun, and Star. Filling them takes up almost the whole of Grace’s time in the building, because each Quartz is locked inside a separate Puzzle Box, and each Puzzle Box needs a different item or ability before you can even touch it.

Quick answer: Open the Chairman’s Office box with Moon, Sun, Star, Moon; the Lead Researcher’s Office box with Star, Sun, Moon, Sun; and the Security Manager’s Office box with Star, Star, Moon, Sun, which only Emily can enter because it is written in braille. Then place all three Quartz in the Central Hall door.

Every code is a four-symbol sequence entered in order on the box dial. The symbols tie back to the three staff members named on the pamphlet in the Guard’s Office: the Chairman, the Lead Researcher, and the Security Manager.

Note: knowing the codes early does not skip anything. The second box is behind a sturdy padlock, and the third box has no visible symbols at all, so both still require the items and events below.


Moon Quartz in the Chairman’s Office

Take the West Wing Keycard from the corpse in the Medication Room, below the Guard’s Office, and open the West Wing. Head left toward the Kitchen and provoke the Zombie Chef so he knocks the cart aside, which clears the double doors on the far side.
Work your way up to the second floor and enter the Chairman’s Office. On the table in the front room you will find a Torn Journal with a page missing, plus a locked Puzzle Box and a Unicorn Trinket worth grabbing.
Deal with the zombie in the back room and take the Pencil from the desk there. Return to the journal and shade the blank page with it to bring out the impression of the code.
Enter Moon, Sun, Star, Moon on the Puzzle Box. It opens and hands over the Moon Quartz. Examine the Unicorn Trinket in your inventory afterward to pop out a Red Jewel, which unlocks the unicorn door in the West Wing hallway.

Sun Quartz in the Lead Researcher’s Office

Use the Red Jewel on the unicorn door to reach the Parlor, then take the East Wing Keycard from the corpse at the back of the room. That opens the East Wing, where the Examination Room holds an organless corpse you will come back to.
Go upstairs in the East Wing and loot the first corpse you reach for the ID Wristband (Level 1). Chunk, a large and slow enemy, spawns in as you pick it up, so retreat downstairs and lose him before coming back.

Enter the Lead Researcher’s Office. A Sturdy Lock blocks the Puzzle Box, and the Lead Researcher’s Message beside it points you to a bottle of Corrosive in the Conference Room on the same floor.
Clear the Conference Room and grab the Corrosive from the back. A Zombie Singer is in here, and its screams wake the other zombies and stagger you at close range, so pull enemies out one at a time or downstairs if ammo is tight. The item is a glass bottle of strong acid that instantly corrodes certain metals.
Head back to the office, watch the cutscene with Victor Gideon, then return once more and melt the padlock with the Corrosive. The three photographs around the desk and sideboard each show part of the dial in the background, which confirms the order.

Enter Star, Sun, Moon, Sun. You get the Sun Quartz and a Leatherbound Notebook titled “Unlocking the Organ Transport Box,” which you need for the next stage.

The organ chain that unlocks the Level 2 wristband

The third Quartz is gated behind security levels, and each wristband is earned rather than found lying around. Here is the access chain in order.

With the Level 1 wristband, open the Custodian’s Office in the West Wing, past the gate near the Parlor and Kitchen entrances. Take the Wrench from the desk and save at the typewriter while you are here.
Go to Cold Storage in the West Wing. The handwheel snapped off earlier, so use the Wrench on the valve to get inside and collect the Artificial Heart.

Enter the Garage next door and pick up the Organ Transport Box at the far end. A zombie driving a construction vehicle crashes in as you try to leave, so shoot the driver to stop the machine and clear the exit.
Examine the box and follow the notebook instructions. Turn the dial to Hand, push the slider beside it up to the second line, flip yellow switches two and five, then turn the dial back to Auto. The lid releases the Artificial Lungs.

Carry both organs to the Examination Room in the East Wing and kill the Zombie Doctor hovering over the table first. Place the heart and lungs into the organless corpse, then put the reanimated body down and take the ID Wristband (Level 2) from it.

Star Quartz, the braille Puzzle Box, and Emily

Use the Level 2 wristband on the security gate in the East Wing to reach the Isolation Ward, then go into the Security Manager’s Office at the back.

Read the journal on the table for the code, Star, Star, Moon, Sun, then inspect the Puzzle Box. Its symbols have been stripped off and replaced with braille, so Grace cannot read the dial herself. Take the ID Wristband (Level 3) from the Security Manager’s corpse beside the note.
Return to the Medication Room below the Guard’s Office and use the Level 3 wristband to open Emily’s cell. Clear the route back through the Isolation Ward before you pick her up, because carrying her blocks combat and slows you down badly.
Carry Emily to the Puzzle Box and set her down. She works through the braille while you defend the room from a wave of zombies, and she calls out to you when the box is open.
Return to her for the cutscene in which Emily is taken away. Control shifts around, including a stretch as Leon, and you finally receive the Star Quartz after finishing the events in the Basement, which include the power distributor puzzle.

Opening the Central Hall door and confirming it worked

Take all three Quartz back to the Central Hall and interact with the door. The objective tracker counts each one as you slot it in, moving from “Find and place the quartz (1/3)” up to three, and the door opens into the Rhodes Hill Courtyard once the third is seated.

This is a one-way exit. You cannot re-enter the main care center building afterward, so finish anything you still want inside first, including Mr. Raccoon Memoriams, Charms, Antique Coins, and the safes in the Examination Room, Bar & Lounge, and Basement. Put Emily down in the Guard Office if you want to explore freely before leaving.


Why a Puzzle Box will not open

  • The symbols must be entered in the listed order. Matching the right set in the wrong sequence does nothing.
  • The Lead Researcher’s box stays sealed until the Corrosive has dissolved the Sturdy Lock, and the Corrosive only appears after the Conference Room is accessible.
  • The Security Manager’s box cannot be solved by Grace at all. Emily has to be carried to it, and the horde fight in the Isolation Ward is part of the sequence.
  • Cold Storage needs the Wrench from the Custodian’s Office because the handwheel breaks off during the earlier escape.
  • The Organ Transport Box will not release the lungs without the dial, slider, and switch inputs done in that order.

Treat the Guard’s Office as your base while you run all of this. It holds the pamphlet that ties each symbol to a staff member, it is where you save between the longer detours, and it is the shortest route back to the Medication Room when it is time to collect Emily.
