The Scalpel is the key item that turns half of Shorefront District — South from scenery into explorable space in Silent Hill: Townfall. Until you have it, the rusty metal doors with a hatch or mail slot in the middle — the St. Amelia Pharmacy entrance among them — simply refuse to open. It sits inside the Shorefront Family Clinic, and the clinic itself is locked behind a chain of objectives at Zoe’s house.

Quick answer: Open Zoe’s organ transport box with 956 (left) and 133 (right) to get her lanyard, badge into the Clinic, and take the Scalpel from the green first-aid kit in the Z Ellis office.


What the Scalpel opens in St. Amelia

The Scalpel is a story item, so it cannot be missed. Its job is narrow and consistent: it cuts through the backing material behind a sealed hatch or mail slot so you can reach in and unlatch the door from the other side. Any rusty door with that hatch in the middle is a Scalpel door.

Keep the Scalpel with you throughout: using it once does not expend it. Opening the park door is especially important, since it leads to the Jess’s house detour and the Broken Stethoscope required for the scales puzzle in Zoe’s office.


Before the clinic: Power, the organ box, and Zoe’s Lanyard

Your starting point is area 5, Town Square, on the Shorefront map. Use the booth there and consult the Local Services flyer beside the handset to make three calls: 900 for emergency services, 010 for the operator, and 100 to save. Completing these calls advances the story and directs you west to Zoe’s house.

Head west from the square and follow the foggy alleyway to Zoe’s building. Climb the outdoor staircase and knock on door 2A. The apartment has no electricity, which blocks the computer you need.

Once outside the alley, head back to Peters Newsagents (area 2). You cannot use the front, so circle around the building and enter through the rear while keeping away from the nearby monster. Search the cash register for the Prepaid Power Card.
Head to Zoe’s again, crouch, and crawl through the gap beside the staircase. Put the power card into the slot at the base of the wall panel. Tap Button A until the display reads Card Credit, then use Button B and toggle the main switch in the panel’s lower-right corner.
With the lights on, switch on the living room computer, refresh the inbox, and read the middle email from Zoe. It contains half of the combination logic for the cooler box in the kitchen.
Go upstairs and into the bathroom attached to the bedroom. The Organ Transplant Card is lying on the floor and supplies the second half of the code.
In the kitchen, inspect the Human Organ in Transit cooler box and set the tumblers to 956 on the left and 133 on the right. Opening it hands you Zoe’s Lanyard Badge automatically.

If the flashlight is still missing, search the drawer positioned to the left of the organ container; upstairs, the second room on the left contains the AA batteries. Combine the two items in your inventory before entering the crawlspace.


Scalpel location: Shorefront Family Clinic, Z Ellis office

The Clinic is area 6 on the map, reached by crossing the town square past the protest signs. A suspended services notice is pinned beside the entrance, and the security shutter is down.

Interact with the electronic card reader on the panel left of the door and select Zoe’s Lanyard Badge. The shutter rises, and the front door can then be unlocked normally.

Walk down the main corridor and take the second door at the end, the room labelled Z Ellis. It is stacked with cardboard boxes, and Zoe’s resignation letter sits on top of one of them.
Open the green first-aid container on the desk beside the computer and collect the Scalpel. It registers as equipment rather than a consumable, so it stays with you for the rest of the chapter.

While you are inside, the clinic holds a few things worth grabbing on the same trip: Gauze from a first-aid kit on the counter in the adjoining room, an Empty IV Bag in a lower desk drawer, and a yellow sticky note on the monitor showing 2896, which is the keypad code for the Reception door. Reception contains the nurse’s rounds form for Zoe Ellis and an answerphone message from the pharmacist, which is what marks the Pharmacy on your map along Main Street.


Using the Scalpel on the St. Amelia Pharmacy door

Leave the clinic and run through the streets to the Pharmacy, area 7 on the Shorefront South map. The way in is the boarded side entrance in the alley, not a shopfront you can walk straight through.

Stand at the rusty metal door and interact with the panel in its middle. The item wheel opens with the Scalpel available as a valid option.
Select the Scalpel and cut away the backing behind the slot. The blade slices through the seal in a short animation and exposes the latch on the inside.
Unlatch the door and step inside. Opening the map confirms your position as St. Amelia Pharmacy — that map update is the clean check that the item worked and that the location is now permanently accessible.

The rusty locker in the pharmacy’s back room takes the code 6539. The digits appear on posters around the shop, and the order matches the sequence the posters show up in on the CRTV broadcast. Inside is the Prescription Dosage Error Record, one of the three items for the scales puzzle in Zoe’s office.


Where the Scalpel takes you next

Once the pharmacy is cleared, the same item handles the blocked door on the south side of St. Amelia’s Park (11). Cutting it open pushes you into the northern stretch of Shorefront South and the Farrow Street houses, where the objective becomes getting into Jess’s house.

  • Take the alley by Number 14 Farrow Street to reach the rear gardens; an enemy patrols here.
  • The note on Jess’s door directs you to Walter at Number 16 for the key.
  • Go upstairs in Building 16, into flat 1/2, and take the 21 Farrow St. – Back Door Key from the table.
  • Unlock the blue back door and take the Broken Stethoscope from the medical box in the bedroom.

With the Broken Stethoscope, the Prescription Dosage Error Record, and Zoe’s Lanyard Badge all in hand, head back to the Z Ellis room and use the scales. Place the stethoscope on the left, the dosage record in the middle, and the lanyard on the right. That closes out the chain the Scalpel started, and the story moves on from Shorefront South.
