Fallout 4 has a new dungeon, and it wants to mess with your head. FEVer Dreams is a free Verified Creation from Kinggath Creations, presented by Bethesda Game Studios. It launched on October 20, 2026, for PC, Xbox, and PlayStation. Its 70-second trailer is mostly green light, klaxons, and a Sole Survivor who can no longer tell what is real.
Quick answer: FEVer Dreams is out now and free in Fallout 4’s Creations menu. It begins when Private Gutsy approaches you near Home Plate in Diamond City’s market.
What the FEVer Dreams trailer shows
The trailer opens with the player dropping into a toxic, green-lit lab as alarms go off. An automated voice reports that chamber integrity has failed and starts counting down to a purge. Mutated specimens float in glowing stasis tanks. A skeleton lies slumped near a terminal marked ANTIVIRAL STATION, which hints at how the last person here fared.
The trailer then cuts away from the lab without warning. One moment the Sole Survivor is wading through decontamination runoff. The next they are standing over baby Shaun’s crib in the pre-war house, and a woman’s voice talks about seeing him again.

The hallucinations speed up after that. The scenery jumps between foggy statues, tables covered in fungus, and a sunny suburban street from before the bombs. These visions run through the whole quest. The FEV infection pulls you back into moments with your spouse, and one of the hallucinated locations is a pre-war Boston Common.

Near the end, the protagonist faces an armored ghoul who points out that only one dose of antiviral is left and asks how they should decide who gets it. The title card follows with “FEVER DREAMS,” then “DO COME TRUE.” The last gameplay shot shows the player holding a green-glowing injector-style pistol in a dark corridor, just before something lunges out of the shadows.

FEVer Dreams story: Camp Jefferson and the race against Pike
The lab sits under Camp Jefferson, a pre-war army base outside the Commonwealth. It was used for Forced Evolutionary Virus research. A Mister Gutsy called Private Gutsy brings you there, insisting it is time for your speech. Once inside, you get soaked in a chemical strain and injected with a dubious antiviral. That leaves you poisoned, and the only cure is somewhere deeper in the complex.
You are not the only one looking for it. Pike, the ghoul soldier from the trailer, wants the cure just as badly and has a head start. Kinggath describes the quest as an escape room-style dungeon. You fight, solve puzzles, and talk your way past security lockdowns and mutated test subjects. Paying closer attention to the puzzles and story unlocks more options and rewards.
Other characters include Dolores and Sgt. Baker. The base is full of notes and holotapes, such as test-subject recordings and a private log from a Dr. R. Kline, that explain what happened there.
Nate and Nora’s original voice actors return
Courtenay Taylor and Brian T. Delaney, the original voices of the female and male Sole Survivor, are back in Fallout 4 for the first time since launch. Each recorded over 1,100 new lines. That much dialogue is what makes the spouse flashbacks work. They give the player their first real conversations with Nate or Nora since the opening minutes of the base game. Wes Johnson and Dave B. Mitchell also return to voice the robots.
FEVer Dreams rewards and Challenge Tokens
Camp Jefferson is packed with prototype gear, and most of it is scattered around the facility. Once you leave the base, you can’t go back. Anything you miss is gone for that playthrough.
| Category | Items |
|---|---|
| Weapons | XC-78 Plasma Caster, Mosquito’s Kiss |
| Armor | Ditanium Plate Armor |
| Clothing | Officer Suit, Officer Hat |
| Named legendaries | Bonk Blocker, Prowler Peepers, others |
| Workshop | Cutouts, including a cat cutout |
The compact plasma caster also appears as the XC-77 in some promotional text. Either way, it is the most expensive purchase in the dungeon’s economy. You find or earn Challenge Tokens throughout Camp Jefferson and spend them at special machines. They buy survival supplies, the plasma caster, or key cards for five Cache rooms, labelled A through E. Tokens are limited, and every machine draws from the same shared stock. Every token you spend on supplies is one you can’t put toward the plasma caster.
FEVer Dreams requirements and start conditions
FEVer Dreams is version 1.1 and takes up about 754.9 MB on PC. It is a free download through the Creations menu and appears under Bethesda Game Studios in-game, even though Kinggath Creations built it. It is designed to run alongside the base game, the DLCs, and many community mods.
- You can use it with a new save or one already in progress, no matter your character level.
- Starts in Diamond City’s market, where Private Gutsy approaches near Home Plate.
- Exit your Power Armor and dismiss your companion before he can take you.
- If you decline, Private Gutsy waits near where you met him until you are ready.
You reach Camp Jefferson with none of your gear. Your belongings are placed by the exit. If you depart without collecting them, they appear in a container close to your wake-up spot in Diamond City.
The quest is underway when Private Gutsy escorts you away from Diamond City and you wake up at Camp Jefferson, unarmed and short on antiviral. From there, it is a race against Pike for the cure while your own memories keep getting in the way.
