Animal Coins are slow to earn in Animal Hospital, so the class you unlock first decides how smooth your night shifts feel. Some classes change a run outright, others barely register once you learn how sanity and anomalies work. Here is where each class sits, what its perk does, and what it costs.

Quick answer: Save for Surgeon if you are free-to-play (sanity plus a short speed boost after every heal). Head Nurse and Secret Agent are the strongest Robux options. Intern and Paramedic are the weakest long-term picks.

The best classes: Surgeon and Head Nurse

Surgeon is the most complete pick and the one to chase if you never spend Robux. Every successful heal restores sanity and grants a short movement speed boost, which becomes an 8-second boost at Level 2 and doubles the sanity gain with +10 bonus sanity at Level 3. Because you heal patients constantly, the perk is working the entire shift. The speed burst matters most in solo runs and during rushes between rooms. The only real catch is the price, since 2,500 Animal Coins makes it the most expensive free-to-play unlock.

Head Nurse is the top option if you are willing to pay. The +3 inventory capacity cuts down trips between the office, patient rooms, and the supply machines, and it scales into +10 bonus sanity at Level 2. At Level 3 you start each round holding a copy of every item, which removes most early setup. It works equally well solo or in a team, but there is no way to earn it without Robux.


Combat and inventory picks: Secret Agent, Security, and Nurse

Secret Agent is the cleanest answer to anomalies you cannot otherwise handle. Starting with a gun lets you remove threats outright, which is why it shines in speedruns and difficult team nights. It costs 320 Robux, so it sits just below the S tier purely on access.

Security is the free-to-play version of that idea. The X-Taser carries fewer charges than the gun, so it rewards careful use, and it is strongest in coordinated teams that occasionally let an anomaly slip through. Nurse is the budget inventory pick at just 20 Animal Coins, making it the best first purchase for new players who keep running out of carry space when patients stack up.


Sanity classes and the weakest options

Doctor, Psychologist, and Secretary all lean on sanity recovery, which is not the strongest lever once you know how to manage the meter. Doctor refills sanity on each heal, Secretary does the same on check-ins at a lower cost, and Psychologist doubles every sanity gain and loss. Psychologist can pay off for experienced players who chain coffee and food items, but the doubled losses punish mistakes, which keeps it out of the top tiers for most lobbies.

Paramedic and Intern bring the least value over a full shift. Paramedic’s Speed Cola buff is short and duplicates an item you can already buy from the Supplies Shop. Intern is the free default, and its starting sanity bonus runs out quickly with no other perk to fall back on. Both are fine to play while you grind toward something better.


How class unlocks and leveling work

You level a class by playing it and earning EXP, which unlocks its Level 2 and Level 3 bonuses over time. Unlocks are permanent once purchased, and you can swap your class in the inter-shift lobby before starting the next night, so there is no penalty for changing picks between runs.
