Sanity is the meter that keeps your character alive in Animal Hospital. It stands in for your mental state, but it works like a health bar, and reaching zero ends the run. You raise it by treating patients, eating food, and stopping skinwalkers, while anomaly attacks and jumpscares pull it back down.

Quick answer: Keep chocolate and coffee in your inventory and eat them when the meter drops. Chocolate restores 60 Sanity total, and each coffee gives 5. Playing as Doctor, Surgeon, or Secretary also refills Sanity as you handle patients.

How Sanity works in Animal Hospital

Every shift throws anomalous events, emergencies, enemies, and anomalies at you, and each one chips away at your Sanity percentage. Because the meter never stops draining on its own, you cannot just react to big hits. You have to top it up steadily across the whole shift so you always have a buffer before the next threat lands.

The safest habit is to carry a few chocolates and coffee cups at all times and keep treating patients between anomaly checks. That way a sudden jumpscare or emergency does not drop you straight to a game-over screen.

Tip: If you keep running out of food, lean on a class that generates Sanity from normal play. The Doctor and Surgeon earn it by healing patients to full recovery, and the Secretary earns it just by admitting patients at the check-in window.

Some Sanity loss is unavoidable and some is self-inflicted. Job Stress ticks away no matter what you do, so treat it as a baseline you always have to out-heal. The rest comes from enemies, jumpscares, and your own mistakes with the Taser.

The two you control directly are photos and the Taser. Stop staring at a cursed or frightening photo the moment you have your answer, and never fire the Taser at a normal patient, since that single mistake costs more than a full Coffee Machine cycle restores.

How to keep Sanity high through a full shift

Start each shift with chocolate and a couple of coffees in your inventory. This gives you an emergency reserve so one bad jumpscare cannot end the run outright.
Pick a Sanity-generating class when you can. The Doctor and Surgeon refill the meter through patient treatment, and the Secretary refills it at check-in, which cuts how much food you burn through.
Deal with anomalies and skinwalkers aggressively. Shooting or electrocuting them gives a small Sanity boost on top of removing the threat, so a Taser or Gun pays off twice.
Cut off avoidable losses. Look away from anomaly photos and the black figure on camera as soon as you can, and hold your Taser fire around genuine patients.
Refill before you dip low, not after. Because Job Stress removes 1 Sanity every minute on top of any hits, topping up early keeps you clear of the zero mark when a Fire or Death Ritual emergency arrives.
