The True Ending is the only outcome in Shift At Midnight where both your pet and Sheriff Clyde walk away alive. It comes down to two things, and neither one has anything to do with how many doppelgangers you shoot. You need to refuse the call to Clyde after Night 12, and you need at least $250 in your own personal funds when the veterinarian asks for the surgery money on Night 13.

Quick answer: Do not call Clyde after Shift 12, keep him alive, and pay the $250 surgery fee yourself on Shift 13 from personal funds. If you finish under $250, the pet dies (Empty Home). If you call Clyde, the surgery is paid but he dies (Grave Decision).


The two conditions that lock in the True Ending

Story Mode runs for 13 night shifts. Your pet gets sick early and survives on medicine for a while, but eventually the vet warns you (after Shift 10) that the illness has stopped responding to treatment. Surgery is the only option, it costs $250, and it has to happen within three days.

The game checks two separate things when the credits are about to roll. The first is whether you called Sheriff Clyde. The second is whether your personal funds hit $250 by the time the veterinarian looks at your balance. Here is exactly what each combination produces.

Here is the trap most players fall into. Refusing Clyde does not automatically save everyone. It only protects Clyde. You still have to cover the $250 yourself, and coming up short sends you straight to Empty Home.


Only personal funds pay for the surgery

Three different numbers show up during a run, and mixing them up is the fastest way to lose the pet. Only your personal funds cover the medicine bills and the operation. Store money never touches them.

Because store spending is its own pool, keep buying every trap and item you need to survive the final Hunt. Skipping defense does not save surgery money, and a bad final night can force a full Shift 13 reset instead. Your personal payout works out to roughly a $10 shift wage plus about 10 percent of store revenue when you beat the quota, minus deductions when you kill a real human. Treat that as a planning estimate, not a fixed guarantee for every shift.


How the $250 deadline and the Shift 13 payout line up

You do not need the full $250 the moment the Clyde choice appears. The payout from Shift 13 is added to your personal total before the vet checks your balance, so a small gap can still close on the final night. The order runs like this: surgery is announced, you earn through Shift 12, you make the Clyde choice, you complete Shift 13, the final payout lands, and only then does the vet look for $250.

That timing makes the last night tight. One successful True Ending run finished at $257 and cleared the $250 fee with only $7 to spare. A large shortfall is a different story. If you are far under heading into the choice, one more shift usually will not rescue you.


Getting the True Ending step by step

Handle the earlier medicine bills as they come up so they do not drain the cash you need later, but do not pay for medicine every single time. Paying keeps the pet available on the next shift, which helps solo players, yet doing it too often leaves you short for the surgery. The moment surgery is announced, treat $250 as your active target.
Protect your personal income across the final three days. Verify each customer before you shoot, since a wrong human kill removes that transaction and docks your pay. Keep the checkout moving so waiting customers do not leave without paying.

When the email arrives after Shift 12 asking you to call Sheriff Clyde, ignore it. Choose Don’t Call Clyde. Reach that choice with $250 already secured, or with only a realistic gap that one clean shift can close.
Play Shift 13 normally and buy the traps, ammunition, healing, and boards you need to survive the final Hunt. Those purchases come out of the store-shopping balance and do not reduce your surgery money. A mysterious creature, The Dentist, shows up during this night; attacking it pulls you into a dream sequence where you run to a room and the creature dies.
After the shift ends and you board the bus, an option appears to pay for the pet’s surgery. Pay the veterinarian directly. That is the action that unlocks the ending.

Note: If you accidentally kill Sheriff Clyde when he visits, you lose the option to call him before the final day, which locks you out of the endings entirely. Leave him alone.


Fastest ways to reach $250 before Night 13

Cash is tight in this game, so start protecting your personal income early rather than scrambling on the last night. These habits add up shift by shift.

  • Search for Tokens at the start of each night before you touch the notice board, since reading it starts the shift.
  • Spin the Token wheel and aim for the $10 landing to boost the day’s revenue.
  • Keep shelves stocked, serve customers quickly, and upgrade the shopfloor to raise satisfaction, which increases what customers pay.
  • Finish Store Manager requests like the rat and cockroach events; clearing the rats in time nets a $20 bonus, and throwing rats in the dumpster pays $3 each.
  • Buy the theft scanner, chase every shoplifter, and make them return items for a cash bonus.
  • Complete a doppelganger’s transaction first, then eliminate them to take their money, unless it is a Blood Moon.
  • Clean up quickly after a fight using the sledgehammer on flesh and the hose on bloodstains, since both are faster than the mop.
  • Throw out trash and rats for small change instead of destroying them; a few extra dollars can be the difference at the vet.

Above all, avoid shooting real human customers. Each wrong kill removes that sale and cuts your final payout, which is exactly the money the surgery depends on.


Deciding whether to skip the Clyde call

The choice lands after Shift 12, before the final night. Accepting locks your run into Grave Decision no matter your balance, so check your personal funds before you decide. Compare the gap you still have against what a normal, clean shift usually pays you.

  • Already at $250: refuse the call. You are safe.
  • Small gap: refuse only if the shortfall is smaller than a recent clean-shift payout. A run sitting at $252 was already safe and turned the offer down.
  • Large gap: a run roughly $75 short could not realistically recover in one shift. If saving the pet matters more than saving Clyde, call him for Grave Decision instead.

How to confirm it worked, and why it sometimes fails

You know the True Ending triggered when the operation succeeds, the pet survives, Clyde still visits, post-story mode unlocks, and the pet becomes available on future shifts. All three possible outcomes resolve right after the vet visit on Night 13, so you will know your ending before the credits.

When the True Ending does not fire, it is almost always one of these reasons.

Chase the True Ending first since it has the tightest financial requirement, then take Grave Decision by calling Clyde, and finish with Empty Home by refusing the call while under $250. Once you know your balance heading into Night 12, the result is entirely in your hands.
