Every mask location, unlock condition, and special effect on the Rex Infernus map, from the Skull Mask to the Twins lightning mod.

Rex Infernus hides four collectible masks, and each one grants a permanent special effect once you claim it. Some hand you outright damage protection, others fold into your ammo mod or free up movement, and every one of them helps you push deeper into the rounds. Each mask sits behind its own side easter egg, so you unlock them one at a time and then swap between them freely at fixed pedestals around the map.

Quick answer: Complete the Skull Mask painting reward in Her House, collect six Mister Peeks parts and beat his grapple trial twice for the Astronaut Helmet, ride Web Mothers for 75 kills and feed one to Pack-a-Punch for the Warden’s Hat, and clear the forest toy-box minigames twice for the Forest Mask. After earning each, re-equip it from its pedestal.

Skull Mask: reject the painting rewards twice

The Skull Mask comes from the loot painting in Her House, and the catch is that you have to stay in the starting room instead of leaving through the portal. If you already left, you can return to Her House through its own easter egg.

Stay inside Her House until Round 8 and melee the painting to the right of the fireplace, opposite Quick Revive. This opens a hidden hatch, and every few rounds a random reward appears behind it.

Ignore the first reward and the second reward, no matter how tempting they are. Rewards can appear as early as one round or as late as four rounds apart, so keep hitting the painting each time you flip the round.
Your third reward will always be the Skull Mask, and you can pick this one up. Because you are trapped in a small room with no perks or upgrades until Round 8, it helps to have Armor, Perks, or the Warden’s Blight ready before you attempt it.

Equipped, the Skull Mask gives you the Stone Cold Stronghold effect. Stand completely still, and a purple ring of fire surrounds you, instantly burning any zombie that steps inside. It only lasts around three seconds before you need to trigger it again, so it works best at the end of a round rather than during hectic fights. You can re-equip it later from a pedestal in Veytharion Sanctuary.


Astronaut Helmet: collect six Mister Peeks parts

The Astronaut Helmet needs the full Mister Peeks body, six pieces in total, all grabbed with the Void Claw grapple hook. The first sits in the upstairs bedroom of Her House, and the other five are placed outside the map boundary.

Grab all six parts with the Void Claw, then make sure the entire map has been opened. Head to Caltheris’s Sanctuary and look above the Mystery Box, on the platform to the right of the main door, where Mister Peeks is waiting.

Interact with Mister Peeks to start his grapple hook time trial, and complete the course in under 60 seconds. You must clear it twice, and the trial can be repeated every round if you run out of time.

The Astronaut Helmet grants fall damage immunity and doubles how long you can hold the Void Claw. Falling off the map into the abyss still kills you, so this does not make you invincible off ledges. Once earned, you can re-equip it in Caltheris’s Sanctuary, directly opposite Mister Peeks.


Warden’s Hat: ride Web Mothers for 75 kills

The Warden’s Hat is tied to the Web Mother elite, which starts spawning from Round 16 onward. You earn it by racking up kills while mounted on one and then finishing the job at Pack-a-Punch.

Use the Void Claw to mount a Web Mother, then kill more than 75 zombies while riding on its back. This usually takes at least two or three Web Mothers across several rounds to complete.
Once you have enough kills, head to the Nexus Forge and use the Void Claw on the Web Mother standing right next to Pack-a-Punch. It jumps in and dies, and the Warden’s Hat drops onto the floor for you to collect.

The Warden’s Hat makes you immune to the slowing effect that webs cause, which effectively neutralizes both Deathspinners and Web Mothers. Those enemies can still deal damage, but they can no longer slow you down. You re-equip it at Nyxara’s Inner Sanctum, which opens after you complete the Warden’s Blight wonder weapon quest.


Forest Mask: clear the Twins toy-box minigames twice

The Forest Mask is the most involved of the four and only becomes available after you have opened Nyxara’s Inner Sanctum during the Warden’s Blight wonder weapon quest. Everything then happens in the hidden forest zone, reached through the tree’s roots.

Each round, interact with the fruit at the tree’s roots to teleport into the forest. Find the tree stump holding a music box and interact with it, then teleport back to the map.
Return to the forest on the next round and a toy box appears between two trees. Interact with it to unlock three minigames, which you can play once per round each.
  • A speed hide-and-seek where you locate the Twins several times in a row.
  • A collection challenge where you grab scattered items while ghostly copies of your character chase and can down you.
  • A jump-rope minigame that ends your attempt if you get hit more than three times.
After finishing all three, head back to the forest and repeat the same three minigames, now in a harder version that glows red. The jump-rope trial in particular throws a slow rope and a fast rope at you at the same time.
Clear all three red minigames, and when you teleport back to Nyxara’s Inner Sanctum the Forest Mask spits out of the tree roots for you to collect.

Note: clear the area around you before starting any minigame. The ghost duplicates in the collection challenge can knock you down mid-horde, and dying to them wastes the attempt.

Once equipped, the Forest Mask activates randomly as you kill zombies, sending a bolt of lightning down onto a horde to electrocute and kill them, similar to Dead Wire, while the Twins dance. This effect stacks on top of your existing Ammo Mod. Its activation timing is inconsistent, so it pays off most when it triggers inside a packed group rather than on a few stragglers. You can re-equip the Forest Mask at Dravakar’s Sanctuary.

Because two of these masks depend on the Void Claw and the Warden’s Blight quest, the practical order is to unlock Pack-a-Punch and the grapple first, work through the wonder weapon to open Nyxara’s Inner Sanctum, and then chase the masks that fit your run. With all four claimed and their pedestals spread across the map, you can switch between damage immunity, mobility, web resistance, or the Twins lightning mod depending on what each high round throws at you.
