How to Use the Sacrificial Heart in Path of Exile 2 for Passive Points

The Sacrificial Heart is a green 2×2 quest item that drops while you explore Act 3’s past in Path of Exile 2. Its tooltip tells you to “sacrifice this heart at the proper ritual site,” but the game never opens a quest entry or points you to that site. The reward is worth the detour, since it hands you permanent skill points you cannot buy or refund easily.

Quick answer: Take the Sacrificial Heart to the Sacrificial Dais in the Trail of Sacrifice inside Aggorat, pick up the Sacrificial Dagger on the altar, then place and stab the heart to receive 2 Passive Skill Points and 2 Weapon Set Passive Skill Points.

What the ritual rewards

Completing the sacrifice grants four permanent skill points to the character that performs it. They are added straight away and can be spent without waiting for a level up. The Weapon Set points are the standout here, because they only come from a small number of quests and special items, which makes each one hard to replace.

Where the Sacrificial Heart drops

The heart is a random drop from Vaal enemies in Act 3’s past timeline, specifically the Vaal Goliath and the Vaal Overseer. You reach this timeline through the Time Travel Gateway in the Ziggurat Encampment, which sends you back to Utzaal (Monster Level 43). It is not guaranteed on the first pack, so kill everything and check the ground carefully for the quest item.

If a full clear does not produce the heart, reset the zone instead of pushing forward into later acts. Stand on a waypoint, open the world map, then hold Ctrl and left-click on Utzaal (or Aggorat) to create a fresh instance with new enemies. Repeat the clear until the heart drops.


Getting to Aggorat and the Trail of Sacrifice

The ritual altar sits in Aggorat (Monster Level 46), the area past Utzaal. Aggorat unlocks after you clear the boss blocking the way forward in Utzaal, the Legion Blockade. Once it is open, you can travel there from the teleport map.

Inside Aggorat, teleport to a checkpoint further up the map and keep moving toward the ritual area. The zone is denser and hits harder than Utzaal, so bring flasks and check your resistances before a long stay.

You know you are on the right path when the map names the subdivision Trail of Sacrifice. Follow it from the starting waypoint toward the altar, which most often appears in the northeast section of the area with a checkpoint beside it.

Performing the sacrifice at the Sacrificial Dais

Reach the Sacrificial Dais and clear the enemies guarding it, including any elite that spawns nearby. You need a safe moment to interact with the altar.
Walk up the steps of the altar and interact with the Sacrificial Dagger to pick it up. This dagger is required to complete the ritual, so make sure it lands in your inventory.

Interact with the altar and choose Place Sacrificial Heart. The heart will settle onto the dais in the center of the site.
Interact with the altar again and select Stab Sacrificial Heart. A short cutscene plays, the heart leaves your bags, and the ritual completes.

Confirming the reward and why the option may be missing

When the sacrifice finishes, the chat window reports “You have received 2 Passive Skill Points” and “You have received 2 Weapon Set Passive Skill Points.” That confirmation is your sign the quest is done, and the points are immediately available on your passive tree.

If the Stab option never appears, the usual causes are simple. Both the Sacrificial Heart and the Sacrificial Dagger must be in your inventory, not on your cursor and not in a stash tab. You also need to be at the correct altar in Aggorat rather than a different quest object elsewhere.


If you already finished Act 3 and the gateway closed

Rushing past Act 3 can close the Time Travel Gateway with the heart still sitting in your bags. You can still reach the altar and finish the ritual by re-linking the past zones.

Travel back to Act 3 Normal town using the waypoint map, then enter the Time Travel Gateway from the Ziggurat Encampment again.
If the past zones still do not show, right-click the Ziggurat Encampment waypoint on the map and reset that instance, then step through the gateway once more.
On Cruel difficulty, go back to Act 3 Normal, pass through the gateway, then advance toward Act 1 Cruel town. Open the waypoint map, where Aggorat should reappear as a direct destination once the past zones are relinked.

Two passive points and two weapon set points are one of the earliest permanent power gains available in Act 3’s past, so it is worth pausing to grab them before you leave. Plan where they go on your tree, since undoing the allocation later is not cheap.
