The patient puzzle near the end of Chapter 7 in Silent Hill: Townfall looks like it should demand a code, but it never asks for one. Once you place the endotracheal tube, the game switches to a short quick-time event built entirely around timing. The only real requirement beforehand is the Medical Intubation Kit, which sits in the basement of St. Amelia Medical Centre.
Quick answer: Collect the Medical Intubation Kit from the Victoria Ward Basement, then hold the insert input each time the white ring bounces into its target. The three positions land on the right, the lower-left, and the top.
Get the Medical Intubation Kit from the basement
You cannot start the intubation without the kit, and the game will not let you skip ahead. It is stored in the Storage room on the Victoria Ward Basement floor, and reaching it means clearing hospital access first and slipping past a roaming figure.
Head to Reception, just to the left as you leave the surgery room, and use the telephone. Dial 5353 to open Hospital Storage. This is the number Dr. Glenn gives you over the intercom.
Take the elevator down and press the basement button. Watch the corridor when the doors open, because a pale humanoid entity patrols the hallway near the storage door.
Move through the kitchen rather than walking straight at the figure. Slip in by the counter and use the door at the back that leads toward the storage area.
Enter the Storage room with the skeleton model and metal shelving. Squeeze through the shelves on the right to reach the wall locker. A hand lunges out between the shelves as Dr. Glenn talks over the intercom, but it is a scare, not an attack.
Open the wall locker and take the Medical Intubation Kit. Then leave the room, use the kitchen and its radio to distract the figure again if needed, and return to the elevator to head back up to the surgery room.
Back at the operating table, interact with the patient to begin. A red target circle appears on screen with a white ring that pulses and bounces inward. The whole puzzle is reading that bounce. When the white ring lines up with the target, hold the insert input to lock in the action. There are three phases, and each one moves the target to a different position.
Start by opening the mouth. Watch the white ring on the screen’s right side; when it bounces into alignment, hold insert. Dr. Glenn narrates every stage, and his cues provide an extra confirmation.
For the final ET tube placement, the target moves toward the top. Time the last bounce, hold insert, and Zoe advances the tube through the cords before removing the stylet.
The positional order stays the same on every difficulty. If you miss a bounce, the prompt simply repeats, so there is no penalty for waiting a full cycle to be sure of your timing.
How to know the puzzle is done
After the third hold registers, Zoe removes the stylet and the sequence resolves on its own. Dr. Glenn confirms the intubation with a line about it not being so hard, then Zoe asks what comes next and he moves the scene toward a median sternotomy. Once that dialogue starts, Chapter 7 is complete and control passes to the next chapter, so there is nothing left to input at the operating table.
