Lacra is the one vampire romance Coen can pursue in The Blood of Dawnwalker, and she is not won over by flirting. Her arc runs across five quests, and the relationship only opens up if you handle a captive humanely, defuse a standoff instead of escalating it, and answer her honestly when she starts talking about her own past. The payoff happens deep in a cavern at the end of The Night of Horrors, and it hinges on a single item choice.

Requirements before the Lacra romance is available
Coen does not know Lacra exists at the start. Her introduction is gated behind Vicho’s quest line, A Study in Crimson, which ends with him mentioning another vrakhiri operating in Svartrau City. That conversation unlocks A Friend Like This. Killing Ambrus also works, since his body carries a note pointing to her.
Note: Lacra is a vampire, so every one of her quests can only be started at night. If a marker refuses to trigger, check the clock before you check anything else.

Hive and Seek: A required quest with no romance dialogue
This one is a straight dungeon crawl. You push deep underground, clear waves of kobolds, and leave with the carcass of Isbrand, the older vampire who sired Brencis. Nothing you say here moves the relationship in either direction, but the quest has to be completed to reach the ones that do.
While you are down there, check the chest to the right of Isbrand for Isbrand’s Plate, a legendary medium chestpiece with 291 armour, +23.4% maximum vampiric health, and +27% blood drinking restoration.
Our Rotten Roots: The fork in Anachora and Isbrand’s essence
Our Rotten Roots takes you into Anachora to walk through Isbrand’s memories. In the third major stretch you hit a fork. The right-hand path leads toward Svartrau City and its castle and is the faster, safer route. The left leads to a pit tied to Lacra’s own past, and she will tell you to stay away from it.

Taking the right path avoids the friction entirely. If you insist on the left instead, Lacra walks off temporarily and you see the memory of what happened to her. Keep going until the game teleports you to the false Svartrau area, then patch things up with “I’m sorry for going there,” “I needed to know what you were hiding,” and “I’m still sorry.” Asking her about the memory afterwards gets you nothing, since she brings it up on her own terms later.
After Isbrand is beaten you decide who takes his essence. Choosing “I’ll do it. I invite you in, Isbrand” keeps it with Coen. Once the quest wraps and you are alone with her in the cave, stay and keep talking about the plan rather than leaving right away, and make it clear you intend to keep helping her.
The Night of Horrors: The quest where the romance happens
The Night of Horrors becomes available roughly a day after Isbrand’s memories are done, and it unlocks alongside Midnight Reckoning. Do not touch Midnight Reckoning first. That quest leads into the finale against Brencis and is a point of no return.
The quest has a hard time limit built into it. Fighting the boyar patrol that spawns near the opening cutscene advances the clock, and if dawn arrives, The Night of Horrors fails outright. When you see soldiers, run past them and leave them alive.

All dialogue choices for the Lacra romance scene
Lacra points out a mandrake growing on the cavern floor and explains what it does. This is the single decision the whole arc rests on. Letting her eat it, or refusing it, ends the night quickly and quietly with no romance.

After the mandrake takes hold, follow her and get through the short exchange about Coen’s family. She then cuts her wrist and invites him to have some fun. Drink, bite her neck, and when she asks what would feel better than this, take the kiss option. The scene plays out, and Coen wakes alone on the cavern floor afterwards.
Rewards and how to confirm the romance triggered
You know it worked when the kiss prompt resolves into the scene rather than the conversation closing out. If the night ends abruptly right after the mandrake conversation, the choice was missed and the quest moves on without it.
Most of the rewards from The Night of Horrors are not tied to the romance at all. Eating the mandrake grants the Mandrake Ward perk, which saves Coen from lethal damage once per night and restores three health segments. The Mesmerize vampiric skill also unlocks either way, and skipping the romance simply hands you a vial of Lacra’s blood you can drink to get it instead. Completing the quest awards the Misery Loves Company trophy.
The romance itself is a one-time encounter. It does not lock you out of Anca, and it does not change your ability to bring Lacra into the final fight against Brencis, though Coen can look back on the night if she stands with him there. Lacra is blunt about it afterwards, telling him that thinking about the future only leads to heartbreak.
