Heartbloom is an optional Side Quest set inside the Simulacrum Nexus of Mengzhou. You help an NPC who tends the Heart of Bloom track down the cause of an abnormal fluctuation, working your way through a chain of flower trails and hidden barriers. Finishing it hands over Astrites, upgrade materials, Shell Credits, and a trophy.
Quick answer: Start at Mengxuan Vale, grab the Realm Key from the Artisan Autopuppet, then use it at each flower vase to extend the trail until you reach the final mechanism and talk to the young man.

Requirements to start Heartbloom
The quest opens as soon as you unlock the Simulacrum Nexus map. Travel to Mengxuan Vale and talk to the NPC by the tree to start it.
After talking to the caretaker, travel to the next marked location and find the Artisan Autopuppet. Pick up the Realm Key here, since it drives every puzzle in this quest.
Move to the anomaly’s location and look for an old man nearby. Interact with him. He ignores you and keeps muttering to himself, so the conversation goes nowhere for now.
Search the surroundings, then check behind the rock for a flower vase. Activate the Heart Realm Key beside it to create a flower trail; follow the blooms and cross the invisible barrier they uncover.
After the conversation, follow the trail through the next invisible wall to reach another vase. Use the Realm Key to extend the flowers again and trace them to the NPC waiting ahead.
Pass through the barrier once more to find a mechanism set into the rock. Place the Realm Key on it. Nothing happens, so take the key back and keep going.
Follow the frequency and the quest marker to the next mechanism. Place the Realm Key here to make a small tree sprout, though it still doesn’t fully resolve the anomaly.
Take the key for the final time and cross the barrier to reach the last mechanism. Place the Realm Key there, step outside the invisible barrier, and talk to the young man to finish the quest.

Heartbloom rewards
Once the final conversation ends, the quest is marked complete and the following rewards land in your inventory. You also unlock the Dream of Nanke trophy for finishing it.

The clearest sign that everything worked is the quest disappearing from your active log and the Dream of Nanke trophy popping after you leave the barrier and finish talking to the young man. If the flower trail ever seems to dead-end, backtrack to the last vase and confirm you used the Realm Key on it before moving forward.
