The Rain Shift is a special weather event in Animal Hospital, the Roblox survival horror clinic game where you check in patients, treat their ailments, and reject disguised anomalies. When it starts raining, the usual deer and bunny patients stop arriving and the front desk fills up with fish-like visitors instead. The rest of the loop stays intact, but the change in patients and the puddles it drops on the floor both matter.
Quick answer: The Rain Shift can begin on any shift from 3 to 8, and you are guaranteed to see it at least once per run. Watch for the on-screen line “It starts to rain outside,” admit the fish patients while rejecting any that look wrong, and step on the puddles to gain Sanity until the rain stops.

When the Rain Shift triggers
The event never shows up at the very start of a run. Much like the Hazmat Team event, it has a chance to fire on any shift between Shift 3 and Shift 8. You will experience it at least once during a playthrough, and it can also repeat more than once across a longer run.
You will know it has started the moment the message “It starts to rain outside” appears. Along with that prompt, you will hear rain hitting the windows and see the ceiling begin to leak, forming puddles across the floor. Those puddles are harmless. Unlike slime, they do not slow or hurt you or your patients.

Add to Google Preferences →Fish patients and Sanity from puddles
While the rain falls, every patient that arrives is a fish-like character, which breaks the normal rotation of deer and bunny visitors. Lore characters can still show up as well, including Ratthew, Barney, and Ron from Accounting, and those can be admitted safely.
The puddles are the main upside. Walking over a puddle grants a single Sanity point, shown in-game as “+1 Sanity (Rain).” If you are playing the Psychologist class, each puddle you step on gives 2 Sanity instead. Since a shift ends the instant your Sanity hits zero, these free points are worth grabbing as you move between the front desk and the medical rooms.

Spotting anomalies among the fish patients
The Rain Shift does not add any unique enemies, but anomalies can still disguise themselves as the fish patients. That means the same verification habits apply. Compare each arrival against its reference photo, and use the security camera feeds to catch odd behavior or physical features before you admit anyone.
If a fish patient asking for a checkup looks like an anomaly, close the shutter to turn it away, or deal with it directly using a Gun or Taser. Everything else works as usual. You can keep checking in normal patients and curing their illnesses while handling any anomaly attacks that occur.
How the Rain Shift ends
Once the event runs its course, standard shifts pick back up. The sound of rain against the windows fades out, and the puddles scattered around the hospital dry up and disappear over time. Regular patients return, and the loop returns to its usual mix of animal visitors.
Note: Because the rain grants extra Sanity with no unique threats attached, the event can work in your favor if you make a point of stepping on puddles. Just do not let the change in patients lower your guard, since a disguised anomaly can still slip through if you skip the photo and camera checks.
