Fratellanza is the second and final chapter of the Man of Honor expansion for Mafia: The Old Country, and it runs noticeably longer than L’Immacolata. The chapter moves through three distinct stages: a heavy firefight inside Alfio’s Sulfur Mine, a truck escort back to friendly territory, and a long stealth break-in at the Lodge that closes out Enzo’s work with Salieri.
Quick answer: Escort Salieri through Alfio’s Sulfur Mine, survive the shootout and the truck ride, deliver the sulfur to Pasquale, then sneak into the Lodge, hold the Baccarat table at gunpoint during the robbery, and drive away from Lodge security to trigger the closing cutscene with Don Torri.
Meet Cesare, take the note, and reach the logging camp
The chapter opens straight after L’Immacolata with a single objective: meet Cesare outside. Move through the opening area, where Enzo runs into Flavio and receives a note. Once that exchange is done, continue out to the garage and grab a car. The yellow tracker marks both the vehicle and your next destination, as it does throughout the game.

Drive to the logging camp where Salieri is waiting. Pull up and let him climb into the car before setting off again. From the camp, follow the yellow marker to Alfio’s Sulfur Mine.

Alfio’s Sulfur Mine shootout
At the mine, follow Salieri inside. A guard stops the group at the entrance and then waves them through. Keep following Alfio’s guard deeper into the tunnels until you reach a large blue gate. A cutscene plays there as Alfio arrives with his men, and the gunfight starts the moment it ends.
The combat is not complicated, but it rewards patience over rushing. Work through it methodically:
- Move from cover to cover and clear guards as they appear.
- Throw Dynamite when enemies bunch up or hold a fixed position.
- Push Alfio forward as the fight advances, using the yellow tracker to stay on route.
- Pick up pistol ammo along the way, so running dry is rarely a problem.
- Grab drops from downed enemies and heal often.
Partway through, a trolley cart loaded with stones needs to be pushed forward. Just after that, an ally ignites a red tanker that explodes and buys a short pause in the fighting.

Alfio is caught soon after, triggering a cutscene. During it, Salieri kills Alfio and takes a special coin-like token. Keep that token in mind, because it matters again at the very end of the chapter.
Protect the truck and deliver it to Pasquale
After the cutscene, Enzo gets into a truck that has to survive the drive out. Throw Dynamite at intervals to keep attackers off the vehicle and hold the incoming damage down. Steady, well-timed throws make this stretch straightforward.

Once the truck is out of danger, regroup with Salieri and follow the yellow marker to Pasquale. Speak with him on arrival to close out the delivery. Visiting Isabella is an optional side stop here before you continue.
Meet Cesare at Bar Caffe Ginevra
Get back in a car and follow the yellow tracker to Bar Caffe Ginevra. Head inside, and a cutscene sets up the second half of the chapter. Everything from here is a stealth operation.
Approach the Lodge through the forest
The Lodge break-in starts with a cutscene during a car ride. When the vehicle reaches a set point on the road, get out and continue on foot. Use the forest path to close in on the Lodge without being seen. The route involves some climbing, a few narrow wall passes, and rough ground, but the yellow tracker keeps the direction clear the whole way.
Sneak onto the Lodge grounds
Near the perimeter, stealth is the only way through. The same stealth systems from the main Mafia: The Old Country campaign carry over here. Stay patient and lean on these tools:
- Crouch to stay low and move quietly between cover.
- Use stealth kills when a guard is within reach.
- Throw knives to drop guards silently from range.
- Drop or throw items to pull guards away and open a path.
- Keep Cesare close and never let the two of you split up.
Follow the yellow marker across the grounds, thinning out guards and creating distractions as needed.
Climb to the balcony and enter the house
At the target house, Cesare boosts you up to the balcony. Scale up, reach the railing, and go through the door. An enemy is standing right inside, so open with a stealth kill from behind before you push any further.

Find the cellar without being spotted
The next goal is the cellar. Open the doors and keep crouching as you move ahead. Timing carries this section, so watch guard patterns and wait for the gap before committing to a move. On the way down toward the gate you need to open for Cesare, a waiter appears. Drop him with a thrown-knife stealth kill, open the gate, and collect the item.

With both of you inside, a waiter shows up again. Follow him quietly and take out any other guards in the area with stealth kills. The yellow marker keeps you pointed the right way.
The Baccarat room robbery
The route leads to a room with a Baccarat game underway. Opening the door triggers a cutscene showing the players around the table, Salieri among them. This is the robbery setup.
While Salieri and Cesare head back through the building for the loot, your job is to stay put and keep the players in line with a pistol trained on them. When gunshots ring out, that is your signal to move to the safe. Follow the yellow marker and use cover to clear any enemies on the way.
Escape the Lodge and finish the chapter
On the way back out, a cutscene hits and a Dynamite blast sends Enzo flying. A firefight starts right after, so use cover to shoot your way through to the front door. Another cutscene reveals a large group of enemies waiting outside, with Salieri and Cesare briefly leaving Enzo surrounded before Cesare returns to help.
From here, the goal is simply to reach the car and drive off:
- Fight through the crowd and heal steadily as you push forward.
- Get into the car and drive away from the Lodge.
- Lose the Lodge security that chases you down the road.
Once you shake the pursuers, a final cutscene plays with Don Torri. He is clearly unhappy with how the job turned out and takes the bulk of the money before leaving. That cutscene marks the end of Fratellanza, which also brings the Man of Honor story to a close. If the chapter completes and the game returns you to Free Ride, the DLC campaign is finished.
