Nyxara’s Sanctuary stays sealed until a laser sits on one exact point on the south wall and an Astral Flame hits her forehead while it is raining. Miss either beat and the ritual simply does not finish.
Quick answer: Repair the Nexus Forge, aim its blue beam at Nyxara, burn the Ancient Scroll, crush the misted zombie in the Titan Trap, lock the dial laser low on the south wall, then throw Astral Flame at her forehead in the rain.

Nyxara cleanse requirements
The ritual will not start until the Nexus Forge is repaired and a few later-quest tools are already in your hands. Get these in place first so you are not running the map mid-step.

Nexus Forge alignment for Nyxara
The Forge has to fire a blue beam into Nyxara’s dragon head before any of the chamber work counts. That beam comes from three cranks on the inner, middle, and outer pillars around the Forge.
Interact with each crank until the three pillars line up on Nyxara’s temple. Keep turning until a blue beam locks into the dragon head. That lock is the start condition. If the beam is on another temple, you are not on Nyxara yet.

Ancient Scroll and Titan Trap crush
The scroll is random, but the drop rule is fixed. Shadow Rift has to be on your gun, and only kills that actually trigger the mod can spawn it.
Put Shadow Rift on your weapon and farm kills in Nyxara’s Inner Sanctum. That room keeps spawning zombies, so the scroll usually appears faster there than in the rest of the map. Pick it up as soon as it drops.
Carry the Ancient Scroll to Nyxara’s Main Chamber and place it in the stone brazier at the center. Leave it there. The next actions all feed that same brazier.
Use the Void Claw to pull a purple orb into Pack-a-Punch, then rotate the Forge cranks until the braziers burn purple. Do not walk into the orb. Direct contact can break it into Salvage and force you to find another one.
Take an Astral Flame from those purple braziers and throw it at the scroll brazier. A red mist will enter a nearby zombie. That marked zombie is now the only one that matters.
Escort the misted zombie to Veytharion Passage and stand it in the middle of the Titan’s gauntlets. Buy the Titan Trap. The zombie should walk under a gauntlet and get crushed. A Nyxara quote playing is the check that this step worked.

Laser beam dial puzzle (south wall)
The dial is the step most runs lose time on. The beam has to sit low on the south wall, toward the far bottom-left, and “close” is not enough. If the spot is even slightly off, souls will not crack the wall.
Stay down until the plate slides away and shows the dial. That can take the rest of the current round and the start of the next one. Standing up early leaves the dial hidden.
Shoot the inner rings to swing the beam, and use the middle ring for small nudges. Stop only when the laser is low on the south wall in that far bottom-left pocket. Treat this as a pixel-perfect lock, not a rough aim.
Kill zombies directly under the dial so their souls feed the beam. A correct aim cracks the wall and reveals a stone head. Shoot that head to spawn four Shadow Souls around the Main Chamber.
Kill every Shadow Soul. They take extra damage from Light Mend and Warden’s Blight, and they can appear above or below the temple, so check both levels. When all four are down, keep shooting the stone statue until the screen flashes white.

Rain and Astral Flame on Nyxara’s forehead
The white flash moves you into the outdoor finish. Two things still have to be true at once. Her eyes must already be purple, and the map must be raining.
In Nyxara’s Passage, pull a purple orb with the Void Claw until it sits directly under her dragon head. Fire a charged Warden’s Blight shot so the orb flies up. Her eyes glowing purple is the check for this throw.
Force rain by interacting with the blue well at the finished Veytharion blocks puzzle. Rain starts on the following round. The forehead throw will not complete in clear weather.
Return to the Nexus Forge for a fresh Astral Flame and a Void Claw. Grapple high in Nyxara’s Passage and throw the flame at her forehead. The throw is easier if the flame floats down onto the target instead of slamming in from the side.
Watch for lightning to strike her head. That strike is the cleanse. If nothing happens, the flame missed, it is not raining, or her eyes never turned purple.
