The Giant Piranha is the main boss of the second island in How to Fish, and beating it is the only way to earn the coordinates that take you to Island 3. It is a fast, cluttered fight where a swarm of smaller piranhas appears alongside the boss, so the trick is keeping steady ranged pressure on the main target while clearing just enough space to survive.

Quick answer: Give three Leeches to the woman beside the lake to receive the Modified Leech, equip it on your rod and reel it in to spawn the boss. Fight in the open with a pistol or shotgun, move sideways when it lunges, eat dead piranhas to heal, then pick up the Giant Piranha Skeleton and hand it back to the woman to unlock Island 3.


Summon the Giant Piranha with the Modified Leech

The boss does not roam the lake freely. You have to bait it out, and the bait comes from the Island 2 leech quest. Until that quest is finished, there is nothing to fight.

Collect three Leeches from the Forest Island and give them to the woman sitting beside the lake. She converts them into the Modified Leech, the bait needed to trigger the encounter.
Equip the Modified Leech on your fishing rod and cast it into the lake. Reeling it back in pulls the Giant Piranha onto the island and starts the fight immediately.

Because the fight begins the moment you reel in, treat the bait as a single limited attempt rather than something to test. If the quest log still shows the collection step, you are not ready for the boss yet.


Prepare your loadout before you cast

You will not have time to organize your inventory once the Giant Piranha appears, so set everything up first. The NPC’s warning to bring a gun is worth taking literally, because this fight is far easier when you can deal damage without standing next to the boss.

Eat before you start so you do not enter the fight hungry, and equip your weapon in advance. A pistol works well as your main damage source, and a shotgun is a solid alternative when the boss closes in. Brass Knuckles are useful for swatting nearby piranhas without burning shotgun ammo, and Dynamite is an optional burst tool for the crowd.

If combat has been the thing stopping you, spending on the 150 Shotgun before the 230 Big Motor is the better call. The faster boat does nothing to help you kill the boss or clear the quest, so put money into the fight first and save boat speed for after Island 3 is unlocked.


Fight the boss with movement and ranged pressure

Stay in the open part of the island where you have room to circle. Unlike the Spider Crab, the Giant Piranha never gives you a clean stun window, so there is no single safe moment to unload. Instead, fire whenever you have a clear line and keep repositioning.

When the boss jumps or lunges at you, move sideways across its path rather than backing straight away. Retreating in a straight line lets the boss and the smaller piranhas collapse the same space around you, and that is how you get cornered and killed.

Keep moving even while reloading. Standing still to line up one perfect shotgun blast usually costs you a free bite, so create distance, reload, then return your aim to the boss. Your rhythm should be simple: let it commit to an attack, sidestep to open an angle, fire, and reposition before it closes the gap again.


Handle the smaller piranhas and heal off their corpses

Several smaller piranhas spawn throughout the fight. Individually they deal little damage, but a full pack can overwhelm you quickly. The mistake is turning to chase every one of them, which stretches the fight out and gives hunger and stray bites time to add up.

Only clear the piranhas that are directly blocking your movement or your line of fire, then return to the boss. You do not need an empty arena to make progress. As a bonus, you can eat the smaller piranhas you kill to restore health or hunger, so treat their corpses as free healing whenever you drop low.


Watch the escape timer under the health bar

The white bar beneath the Giant Piranha’s health is its escape timer. If it runs out, the boss disappears and you have to obtain and use the bait all over again. That is the reason you cannot afford to spend the whole fight dodging without returning damage.

Balance the two demands. Stay mobile enough to survive, but keep putting shots into the boss often enough that its health keeps falling before the timer expires. Do not let the timer panic you into standing still and trading bites.


Confirm the kill and grab the Giant Piranha Skeleton

When the health bar empties, the boss dies and a “New Creature Caught: Giant Piranha” notification confirms it. That message is your sign the combat portion is done and the creature is added to your collection.

Beware that the remaining smaller piranhas can still kill you after the boss falls. Clear or outrun the leftover pack before you relax. Your priority pickup is the Giant Piranha Skeleton, which is the quest item that actually advances the story. Ordinary meat drops around the body are not what the NPC wants, so secure the Skeleton first and sort the rest afterward.


Hand in the Skeleton to unlock Island 3

Killing the boss does not open Island 3 on its own. Return the Giant Piranha Skeleton to the woman beside the lake to complete the quest. She rewards you with the coordinates for the third island. Head back to your boat and check the Radar for the new marker, then set off.

Once Island 3 is unlocked, the Big Motor becomes a more reasonable purchase, since boat travel is now part of your route rather than a distraction from the fight.


If the fight keeps failing

Change one thing before spending another bait rather than repeating the same setup. Use the symptom to decide what to fix.

Follow that order and the Giant Piranha stops being a wall. Beat it once, hand back the Skeleton, and the path to Island 3 opens the moment those coordinates appear on your Radar.
